Position Summary:
As a member of Foothills Country Hospice Society (FCHS), this individual- an experienced Licensed Practical Nurse, preferably with palliative experience, provides direct care to residents and families with guidance from Registered Nurse. The Licensed Practical Nurse will work as part of the clinical team, to provide excellence in palliative care in accordance with established policies, procedures and standards to ensure resident and family safety is a priority.
Position Responsibilities Include:
- Providing direct resident care in a team environment and demonstrating excellent pain and symptom assessment skills with timely management within the Licensed Practical Nurse (LPN) scope of practice.
- Consulting and collaborating with members of the care giving team such as; Medical Director, Physicians, Pharmacists, Clinical Manager, Registered Nurses (RN) Health Care Aides (HCA), Social Worker, Volunteer Coordinator and Volunteers in order to provide safe, high quality hospice palliative care for residents and their families.
- Working collaboratively with the RN, who will coordinate the care plan with the LPN HCA, volunteers and other members of the interdisciplinary team to provide care consistent with the Hospice's Mission, Vision and Values.
- Participating in rounds with multidisciplinary team members.
- Working collaboratively with the resident, family, and care giving team to develop, implement, and evaluate the resident and family care plan.
- Providing ongoing resident assessment and evaluation of physical, psycho-social and spiritual needs.
- Documenting relevant information in a timely and accurate manner.
- Being a positive role model for other employees, volunteers, residents and families.
- Participating in bereavement support for families.
- Completing required learning modules, competencies, and annual self assessments.
- Assuming other nursing related duties as assigned by the Clinical Manager.

- Reporting Line: Clinical Manager
- Hours of Work - Various Permanent Lines: 0.5 FTE
- Shifts: Day/Night & Evening/Night
Minimum Requirements:
- Current C.L.P.N.A. (full scope of practice) registration or eligible to receive C.L.P.N.A. registration.
- Current BLS Health Care Provider CPR.
- Preference will be given to individuals with palliative care nursing experience/training.
- Willing to participate in educational advancement within the palliative care field.
- Availability to participate actively in team meetings and quality improvement activities.
- Knowledge about, and committed to, the Hospice Mission, Vision and Values.
- Awareness of one's own limitations and strengths in emotionally sensitive situations and ability to react calmly and effectively.
- Ability to represent the Hospice in an appropriate manner.
- Ability to be flexible, work well in a new and changing environment and promote progressive/innovative thinking as well as team-based care.
- Have knowledge of, and be sensitive to religious, ethnic and cultural diversity.
- Must have good communication skills both verbally and non-verbally and the ability to deal with people in crisis in a mature and non-judgmental manner.
- Must have the ability to maintain a feeling of peace and comfort in the Hospice.
- Ability to recognize the need for self-care in the Hospice workplace.
This position requires the individual to be compliant with all FCHS privacy and confidentiality policies and procedures; and to be compliant with all Privacy Legislation (FOIP, HIA, PIPA) relevant to FCHS.
