Overview and Purpose of the Role
As a Nurse Navigator you will promote a patient- and family-centered approach in advancing the delivery of comprehensive, high-quality, and safe care at Schroeder Ambulatory Centre. Guided by evidence-based nursing practices and SAC standards, you will work both independently and collaboratively within the interprofessional care team to cultivate an outstanding care experience for patients and their families. The Nurse Navigator provides comprehensive support throughout the procedure/ surgical patient journey from assessment and treatment referral to recovery and post op follow up, contributing to improved outcomes and satisfaction.
Key Duties & Responsibilities:
- Triage and screen patients pre procedure to ensure criteria is met.
- Ability to to lead, delegate tasks and motivate team members
- Support team intra procedurally as operationally needed
- Manages the day-to-day process in the procedural suites to enable delivery of patient care outcomes.
- Perform clinical oversight by acting as an expert resource for nursing and allied staff, addressing urgent care issues, supporting complex decision-making, and responding to clinical concerns within the procedural area
- Participate in the orientation of new staff by providing support, coaching and mentorship by monitoring, evaluating the progress of their performance and communicate this to their reporting manager.
- Working with the manager and procurement team to ensure supplies & Inventory are available and updated for procedures
- Liaising with MDRD Infection Control team to create efficiencies with reprocessing for procedures
- Oversight of and support clinical staff to maintain readiness of procedural equipment and environments ensuring infection control and safety compliance for appropriate turnover of rooms.
- Inspect and maintain readiness of equipment and clinical environments by checking crash carts, supply carts, and ensuring infection control and safety compliance.
- Interviewing patients and families in preparation for their procedure/ surgery (via telephone interview or in person), sharing any required pre op/ procedure instructions
- Reviewing pre-procedural/ operative orders, including laboratory and ECG tests
- Facilitating consults with anesthesia, medicine, lab technologist, ECG technician, physiotherapist, pharmacy as per physicians' orders and medical directives.
- Providing pre-procedural/ operative health teaching and educational material tailored to patient's care plan and procedural/ surgical pathway
- Coordinates and participates in quality improvement and risk management activities for assigned areas.
- Complete post procedural/ operative follow up calls to ensure satisfactory patient outcome.
- Perform additional duties and undertake special projects as assigned
- Follows up on incident reports and assists to identify and monitor trends and report on findings within the procedural area.
- Computer proficiency in MS Office (Word, Excel, Outlook) and Adobe
Corporate Responsibilities:
- Participate in all mandatory training requirements and work in compliance with the Occupational Health and Safety Act and Regulations, Schroeder Ambulatory Centre Policies and Procedures, as well as established industry guidelines
Education, Skills, & Qualifications:
- Bachelor's degree in healthcare-related field (e.g., nursing, health sciences).
- Current registration with the College of Nurses of Ontario
- Current BCLS and ACLS certification
- 3 years of recent related experience in surgical care, patient flow or patient navigation preferred.
- Strong understanding of clinical best practices and evidence-based research.
- Proficiency in coordinating patient care services and referrals.
- Excellent leadership, collaboration, decision making, problem recognition and problem solving, and critical thinking skills
- Self-directed and highly motivated with excellent interpersonal and communication skills
- Knowledge and Experience with Meditech is an asset

Pay: $92,820.00-$109,200.00 per year
