Job Summary

We are seeking a compassionate and dedicated Caregiver/Personal Support Worker (PSW) to provide exceptional care and support to individuals in various settings, including home, assisted living, group homes, and long-term care facilities. In this vital role, you will assist clients with daily activities, monitor their health status, and ensure their comfort and safety. Responsibilities

  • Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, toileting, and mobility support
  • Monitor patients' health by taking vital signs like blood pressure, pulse, temperature, and respiration; report any concerns promptly
  • Support medication administration following care plans and HIPAA privacy regulations
  • Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary
  • Use specialized equipment such as Hoyer lifts or resident lifts to safely transfer clients with mobility challenges
  • Conduct patient observation to identify changes in condition or behavior, especially in dementia or memory care cases
  • Maintain accurate documentation of care provided, patient progress, and incident reports in compliance with healthcare standards
  • Perform light cleaning duties to ensure a safe and hygienic environment for clients
  • Collaborate with healthcare teams including nurses, social workers, and therapists to develop or update personalized care plans
  • Assist with transportation to appointments or community activities as needed
  • Provide emotional support and companionship to promote mental well-being

Requirements

  • Valid certification in First Aid and CPR from recognized organizations
  • Prior experience working in assisted living facilities, nursing homes, group homes, or home & community care environments is highly preferred
  • Experience working with individuals with developmental disabilities, autism spectrum disorder, Alzheimer's disease, dementia, or other memory impairments
  • Knowledge of HIPAA regulations ensuring client confidentiality at all times
  • Ability to lift heavy objects safely using proper techniques; experience with Hoyer lifts is a plus
  • Strong patient observation skills to monitor health status effectively
  • Excellent communication skills to interact empathetically with clients and team members
  • Ability to work independently as well as part of a multidisciplinary team
  • Flexibility to work various shifts including evenings, weekends, or holidays as required
  • A caring attitude combined with energy and motivation to improve clients' quality of life
