The Owen Sound Family Health Team (OSFHT) is seeking a Full-Time Nurse Practitioner to join our team. The OSFHT is a team of allied health who are committed to improving and maintaining the health of the community. Based on local health and community needs, we provide access to innovative, high quality and comprehensive primary health care programs and services.
The Nurse Practitioner will work within the full scope of practice set out by the College of Nurses of Ontario for Registered Nurses in Extended Class and will report to the Programs & Services Manager, and the Executive Director.
As a Nurse Practitioner with the OSFHT you will have the opportunity to provide high quality primary care, emergency room diversion, chronic disease management and health promotion while working with a supportive and collaborative interdisciplinary team.
DUTIES & RESPONSIBLITIES
· Participate in the Primary Care attachment program, assisting with program design and implementation.
· Participate in emergency room diversion through our Same Day/Next Day clinic providing episodic care.
· Complete health assessments including health history, physical examinations, ordering diagnostic and laboratory tests, formulating and communicating medical diagnosis and disorders, referring and prescribing medications in accordance with the NP scope of practice.
· Maintain positive channels of communication & cooperation with the patients we serve, as well as other team members.
· Participate in the development of OSFHT programs and will work with the multidisciplinary team, patients, and community partners to provide integrated services to meet the health needs of individuals, families, groups, communities and populations.
· Provide feedback to management regarding needs or suggestions for facilitating optimum personal or professional (office) efficiency
· Comply with OSFHT Policies & Procedures
· Maintain a neat and orderly work space
· Adhere to the Health and Safety Policies and Procedures, with particular regard to own safety and that of other people on premises
· Other duties as assigned.
QUALIFICATIONS & REQUIREMENTS
· Current registration with the College of Nurses of Ontario
· Nurse Practitioner (Registered Nurse Extended Class), Primary Care
· RNAO/NPAO membership required
· Three years of demonstrated experience in a primary care or community setting preferred
· Strong interest and experience in program development, implementation and evaluation
· Ability to work effectively and collaboratively within a multidisciplinary team
· Proven efficiency working in a fast-paced environment
· Keen attention to detail and accuracy
· Commitment to best practices and quality patient care
· Able to demonstrate an understanding of PHIPA and confidentiality
· Excellent computer skills with electronic medical records experience
· Experience with Microsoft Office and desktop publishing is an asset
· Proficiency with an Electronic Medical Record System. Experience with Telus PSS is an asset
Excellent work performance and attendance record is essential
Job Types: Full-time, temporary 6-month contract position
Pay: $ 64.35 per hour
Expected hours: 37.5 per week
