This
one-year contract role
offers the opportunity to make a meaningful difference in the lives of patients transitioning from hospital, while supporting a dedicated team of healthcare professionals.

As Clinical Manager, you will play a pivotal role in ensuring clinical excellence, operational efficiency, and strong partnerships with hospital stakeholders. Reporting to the Area Director, Transitional Care Programs, this position is ideal for a healthcare leader who values collaboration, continuous improvement, and compassionate care.

What You'll Do

  • Supervise and support front-line staff to ensure care delivery aligns with provincial legislation, professional standards of practice, and Bayshore policies.
  • Oversee clinical operations, ensuring patient assessments, care plans, treatments, and follow-ups are completed accurately and efficiently.
  • Collaborate with team to implement patient satisfaction initiatives and ensure timely resolution and documentation of patient concerns.
  • Liaise with hospital partners, funders, and government programs to support coordinated care and effective transitions.
  • Lead recruitment, orientation, training, performance management, and disciplinary processes in collaboration with Human Resources.
  • Assist in the implementation of Modified Work Programs for staff returning from leave or requiring accommodation.
  • Interpret and apply policies and procedures relevant to the Transitional Care Unit to ensure consistency and compliance.
  • Ensure adherence to Occupational Health and Safety standards, including conducting inspections, maintaining safety committees, and reporting incidents.
  • Maintain compliance with Public Health standards, Ministry of Health directives, and Bayshore's Infection Prevention and Control protocols.
  • Monitor and report on Key Performance Indicators to evaluate unit performance and identify areas for improvement.
  • Conduct audits and review patient documentation to ensure accuracy, completeness, and alignment with regulatory and internal standards.
  • Maintain databases with patient and staff information, ensuring data integrity and compliance with Bayshore's data quality standards.
  • Collaborate with the Area Director to manage unit costs and ensure financial targets are met.
  • Support business development initiatives and assist in the development of contractual agreements for client care and services.
  • Participate in strategic planning and market analysis to support program growth and innovation.
  • Represent the unit in operational and external meetings, contributing to organizational goals and community engagement.
  • Coordinate cross-training of staff to ensure coverage during absences and maintain continuity of care.
  • Promote Bayshore Integrated Care Solutions through participation in local healthcare committees and community events.
  • Serve as the on-call manager when required, providing leadership and support outside regular hours.
  • Engage in ongoing professional development through internal and external continuing education activities.
  • Adhere to Bayshore's policies and procedures and contribute to continuous improvement initiatives aligned with the Quality Management System.
  • Participate in proactive health and safety activities and ensure timely reporting and documentation of workplace incidents.
  • Maintain confidentiality of client and corporate information, sharing details only with authorized personnel.
  • Demonstrate consistent performance across key job dimensions and contribute to a positive, high-performing team culture.
  • Perform additional duties as assigned to support the success of the Transitional Care Unit.

What You'll Bring

  • A nursing diploma from an accredited program; Bachelor's degree is preferred.
  • A current license in good standing as a Registered Nurse (RN) or Registered Practical Nurse (RPN) in Ontario.
  • 5–10 years of progressive experience in healthcare, ideally in home care, hospital, retirement, or long-term care settings.
  • Completion of Continuing Education Units (CEUs) as required by regulation.
  • A proven ability to build and maintain referral relationships and navigate the Canadian healthcare system, including OHIP and the Public Guardian and Trustee.
  • Strong knowledge of medical terminology, patient care best practices, and infection prevention and control protocols.
  • A leadership style that reflects Bayshore's values of compassion, excellence, and reliability.
  • Experience in discharge planning and working within team-based care environments.
  • Proficiency in Microsoft Office and other business technologies.
  • Strong communication skills and the ability to read, write, and speak English fluently.
