Job Description: The Assessor Coordinator-Degree Senior is a senior clinical, team-lead position providing leadership, coordination, and clinical oversight to support the implementation, evaluation, and ongoing delivery of the System Flow Coordination Center (SFCC) Continuing Care Portfolio. The position is accountable for strategic direction through education, clinical support, implementation, and evaluation activities. This role provides clinical expertise, guidance, and mentorship to front-line staff, ensuring consistent, safe, and effective practices. The position supports complex care planning by ensuring staff have the appropriate resources, training, and clinical supports required. This role acts as a subject-matter expert for the SFCC portfolio. The role will collaborate with other SHA portfolios to support coordinated service delivery. The incumbent provides leadership and advice related to program development, implementation, and evaluation, including supporting and guiding program changes.
