Job Summary

Community Resource Worker- Outreach plays an important role in supporting individuals to live as independently as possible in their homes and community. This position involves assessment and goal planning, provides skill-building support, promotes safety and well-being, and works closely with clients, families, and other professionals to ensure high-quality, person-centered care.

Key Responsibilities

Program Planning & Support

  • Participate in assessment, goal setting, and development of Individual Service Plans.
  • Document, implement, and provide input into program evaluations.
  • Assist with case management by identifying potential problems and reporting difficulties.
  • Contribute to the evaluation of participants' progress and prepare accurate reports.

Daily Living & Community Skill Support

  • Support participants to increase independence in their homes and communities.
  • Assist with daily living skills, social development, and behavior management strategies.
  • Provide life skills training such as meal preparation, housekeeping, personal care skills, and personal finance.
  • Assist with activities of daily living, including feeding, lifts, transfers, hygiene, grooming, and toileting.
  • Perform specialized care tasks as required (e.g., gastrostomy feeding, tracheostomy management, suction, and bowel management).
  • Teach and assist with activities such as grooming, basic cooking, money management, shopping, household safety, and pet care.
  • Facilitate physical, recreational, educational, social, and vocational activities.

Health & Safety

  • Respond promptly to emergencies, including situations involving potential aggression, ensuring safety for all.
  • Report concerns the supervisor as required.
  • Administer medications in accordance with organizational policies
  • Ensure proper infection control and sanitation practices in all environments.
  • Maintain high standards of health and safety in all environments.

Monitoring & Communication

  • Review and evaluate participants' progress and adjust programs as needed.
  • Provide ongoing support and constructive feedback to participants and families.
  • Accompany or transport clients to appointments, shopping trips, and leisure activities.
  • Prepare accurate written and verbal reports detailing daily activities, progress, and required documentation.

Community Connections & Transportation

  • Identify community resources that meet participants' social, recreational, physical, vocational, and educational needs.
  • Advocate for participants by liaising with community agencies and professionals to access necessary resources.
  • Maintain positive relationships with other agencies, professionals, government representatives, and community members.
  • Accompany or transport clients to appointments, shopping trips, and leisure activities; operate vehicles safely and responsibly.

Household Maintenance

  • Perform housekeeping and maintenance duties in supportive living environments, including laundry, sweeping, mopping, lawn care, inventory, shopping, and cleaning equipment.

Other

  • Perform other related duties as required.

Education & Experience

  • Certificate in a related human or social services field.
  • One (1) year of recent related experience, or an equivalent combination of education, training, and experience.

Requirements

  • Class 5 Driver's License (may be required to obtain Class 4).

  • Current First Aid and CPR certification.

  • Current (every 3 years) TB test results

  • Clear and current Criminal Record Check (completed through OneSky).

  • Physician's Approval of Health.

  • Food Safe Certification.

  • Current Safety Care Training.

  • Willingness to swim regularly with participants.

  • Use of a safe, reliable personal vehicle with "business class" insurance for transporting participants.
