The Access to Care leadership team is seeking a full-time, contract (24 month) Clinical Operations Coordination Leader who will be critical in supporting the operation and coordination of both clinical and administrative functions for CAMH Patient Flow. You will work collaboratively with the Patient Flow Manager, Emergency Department leaders, Inpatient and Outpatient leaders, as well as front line staff. You will be reporting to the Senior Manager, Emergency Department, and Patient Flow.

The Clinical Operations Coordination Leader will support Patient Flow protocols. You will oversee and facilitate effective and efficient patient admissions, transfers, and discharges, supporting CAMH patient flow guidelines and processes. This role will involve developing and maintaining positive relationships with internal and external stakeholders to ensure seamless access across the continuum of care.

Your Key Responsibilities Will Include

  • Patient Flow Management

  • Collaborate with team leads, charge nurses, and multidisciplinary teams, including physicians and administrative staff, to ensure timely and safe admissions, transfers, and discharges.

  • Monitor and manage ED volume and direct patient flow between departments and external hospitals.
  • Follow up with inpatient teams on bed capacity, discharge forecasts, and transfer needs.
  • Participate in daily system capacity calls with partnering hospitals to balance regional bed supply and demand.
  • Respond to patient surges and coordinate effective utilization of resources.

  • Strategic Care Planning

  • Match patients to appropriate beds, considering health equity and operational challenges.

  • Review patient charts and collaborate with internal and external teams for care coordination.
  • Facilitate clinical discussions to align on patient care needs and admission/transfer priorities.

  • Communication and Coordination

  • Ensure timely communication and coordination across departments for seamless patient care.

  • Develop and maintain relationships with external healthcare providers for continuity of care.
  • Provide regular updates to hospital leadership and stakeholders on patient flow metrics and challenges.

  • Quality and Process Improvement

  • Contribute to quality improvement initiatives.

  • Monitor and maintain quality and safety standards in patient flow processes.
  • Collect data to optimize patient flow operations and inform process enhancements.

  • General Responsibilities

  • Participate in risk management, patient safety, and quality improvement activities.

  • Maintain compliance with health and safety regulations, professional standards, and hospital policies.
  • Ensure efficient use of facilities and equipment to support patient flow.

You will contribute to promote a supportive/collaborative learning environment based on the principles of adult learning and practice standards; liaising with members of the inter-professional team to ensure high standards of quality and management of patient care outcomes; performing cross-functional; and/or other duties consistent with the job classification as assigned or requested. Job Requirements
The successful candidate must have a bachelor's degree in nursing and be registered and in good standing with a regulated health professional college. Two (2) years of recent, related clinical leadership experience is required. Previous supervisory/management experience is preferred. Knowledge and experience within an Emergency Department, inpatient units and Patient Flow is strongly preferred. You will have demonstrated knowledge and experience of mental health and addiction systems and an understanding of service delivery across diverse patient populations. You will have a proven commitment to supporting teams and clients and family-centered care. Strong interpersonal and communication skills (both written and verbal), and the ability to work effectively within a dynamic inter-professional team are essential to the role. You have proficiency with Microsoft Office (Word, Outlook, Excel, and PowerPoint). Bilingualism (French/English) and/or proficiency in a second language would be an asset.
