Reports To: Nurse Supervisor / Clinical Director
Employment Type: Full-Time/Part-Time/Casual/ Flexible Hours
Primary Work Setting: Client homes (community-based care)
Compensation: Competitive hourly wage (based on experience and scope)
Role Purpose
The Licensed Practical Nurse (LPN) provides nursing care, clinical monitoring, and delegated supervision to clients receiving services through RISE Care. Working independently and collaboratively, the LPN supports clients' health, safety, and functional goals while contributing to RISE Care's personalized, restorative model of care.
The LPN plays a key role in assessment, care delivery, monitoring, documentation, and coordination, and may provide direction and delegation to Health Care Aides (HCAs) and Care Assistants as appropriate.
Core Responsibilities
1. Nursing Care & Clinical Support
- Provide nursing care within LPN scope of practice, including assessment, interventions, and evaluation of client status.
- Deliver care in accordance with individualized care plans, nursing standards, and organizational policies.
- Support clients with both physical and psychosocial health needs in the home setting.
2. Health Assessment & Monitoring
- Conduct focused nursing assessments related to physical health, functional status, cognition, mood, and safety.
- Monitor and document changes in condition, response to care, and emerging risks.
- Escalate concerns and collaborate with RNs, physicians, and other clinicians as required.
3. Medication Management
- Administer medications and treatments in accordance with scope, training, and care plans.
- Monitor medication effectiveness, adherence, and potential side effects.
- Provide education to clients and families related to medications and treatments.
- Communicate medication-related concerns to the supervising nurse or prescribing provider.
4. Restorative & Functional Support
- Support and reinforce restorative care goals, including mobility, functional activities, and routines.
- Encourage independence while ensuring safety and clinical appropriateness.
- Collaborate with the care team to adapt care plans based on client progress or decline.
5. Delegation, Supervision & Team Support
- Provide direction, guidance, and support to HCAs and Care Assistants within established delegation frameworks.
- Participate in delegation, supervision, and follow-up as assigned by the supervising nurse.
- Serve as a clinical resource to team members in the home and community setting.
6. Documentation & Communication
- Maintain accurate, timely, and professional nursing documentation.
- Communicate effectively with the care team, clients, families, and external providers.
- Contribute clinical observations and recommendations to care planning discussions.
7. Client & Family Education
- Provide education related to health conditions, care plans, medications, and safety.
- Support clients and families in understanding care goals and expectations.
- Promote informed decision-making and self-management where appropriate.
Supervision, Scope & Accountability
- Practices in accordance with the College of Licensed Practical Nurses of Alberta (CLPNA) standards, scope of practice, and applicable legislation.
- Exercises professional judgment and accountability within LPN scope.
- Collaborates closely with supervising RNs and escalates concerns appropriately.
- Supports safe, appropriate delegation and supervision of unregulated care providers.
- Maintains client confidentiality and privacy at all times.
Required Qualifications
- Licensed Practical Nurse in good standing with the College of Licensed Practical Nurses of Alberta (CLPNA)
- Current CPR & First Aid
- Criminal Record Check with Vulnerable Sector
- Ability to work independently in client homes
- Reliable transportation (as required for assigned clients)

How This Role Fits at RISE Care
The Licensed Practical Nurse plays a central role in ensuring safe, high-quality nursing care within RISE Care's home-based services. LPNs work collaboratively with RNs, HCAs, Care Assistants, and other clinicians to support individualized care plans, identify changes early, and promote client well-being and independence.

Pay: $25.00-$36.00 per hour
