Sage Meadows Medical Centre, Now, Torrent (Sage Meadows) Medical Centre is seeking a full time Licensed Practical Nurse who is able to work in collaboration with Doctors, Pharmacists and other staff members.

Random visits to our other locations is also required to oversee workflow.

Job Overview

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide essential nursing care to patients in various settings, including nursing homes, medical offices, and hospice care environments. The LPN will work under the supervision of registered nurses and physicians, ensuring that patients receive high-quality care tailored to their individual needs.

Please Note

  • Minimum 3 to 5 Years experience required working in a community Medical Clinic
  • Must have complete knowledge of ACCURO EMR
  • Must be a team player and have an exceptional customer service
  • Must work in collaboration with doctors, Pharmacists and other staff members

Duties

  • Administer medications and treatments as prescribed by healthcare providers.
  • Monitor patient health and report any changes to the supervising physician.
  • Assist in developing and implementing patient care plans.
  • Provide direct patient care, including , dressing, and feeding patients as needed.
  • Maintain accurate patient records and documentation using electronic health record systems such as Cerner.
  • Collaborate with doctors and pharmacists to ensure comprehensive patient care.
  • Help Doctors with minor procedures
  • Uphold safety standards and infection control protocols within the facility.

Skills

  • Proficient understanding of medical terminology relevant to nursing practices.
  • Experience in nursing home settings or hospice care is preferred.
  • Strong case management skills to coordinate patient care effectively.
  • Competency in medication administration techniques and protocols.
  • Excellent patient care skills with a focus on empathy and communication.
  • Familiarity with medical office procedures and laboratory practices is advantageous.
  • Ability to work collaboratively within a healthcare team while maintaining professionalism.
  • Strong organizational skills with attention to detail in documentation and reporting.

Job Types: Full-time, Permanent

Pay: $23.00-$27.00 per hour

Licence/Certification:

  • First Aid Certification (preferred)
  • LPN/RPN (preferred)
  • Nursing (preferred)
  • CPR Certification (preferred)
