Supports and advocates for the rights, respect and dignity of persons with a developmental disability – promoting choice and focusing on ability while being mindful of the barriers clients with developmental disabilities often face. Believes in the principals of integration and inclusion in the community. Works from a strength-based / person centered approach.

KEY RESPONSIBILITIES: In cooperation with the Manager, Supervisor, and other YWCA Hamilton employees, the employee will perform all duties and responsibilities in a productive, professional manner while adhering to departmental and organizational policies / procedures and guidelines – contributing to the overall goals of the department and the strategic vision of YWCA Hamilton. Ensure the physical and emotional well-being of individuals we support is maintained. Meet QAM Compliance and MCSS Legislation 299/10Standards or Schedule 20 By-Laws.

RESPONSIBILITIES:

·       In cooperation with the Program Supervisor this employee will perform all duties and responsibilities in a productive, professional and cooperative manner while adhering to departmental and organizational policies / procedures and guidelines which will contribute to the overall goals of the department and the strategic plan of YWCA Hamilton.

·       Meet QAM compliance and Legislation 299/10 standards.

·       Ensure the physical and emotional well-being of individuals we support is maintained.

·       Provide direct and indirect support services – support services may include, but are not limited to: case-coordination, advocacy, facilitation, administration, recreational planning, promotion of social inclusion, activities of daily living. Support services will focus on enhancing the clients overall quality of life through meaningful activities and promote community participation and social inclusion.

·       Provide therapeutic recreation supports to individuals following Recreation Therapy Standards of Practice within Developmental Services Policy and Procedures.

·       Case Coordination Assistance: facilitating the linkage of support services / referrals / advocacy in order to meet individuals' needs in cooperation with individuals, families, other staff and community service agencies within the service plan domains, in cooperation with Case Coordinators

·       Establish positive professional rapports with staff, clients, client's families or advocates, and other community service agency providers.

·       Provide assistance to participants in opportunities for community participation, socialization, recreation, and therapeutic programming.

·       Complete administration duties as required and as assigned - ensuring written records / documentation and accountability are completed and maintained.

·       Complete accurate and timely reports as required; ensuring all Ministry and other reporting requirements are met.

·       Complete intakes, assessments, and discharge summaries.

·       Facilitate the programming of (ISP) Individual Support Plans and ISP Reviews and implement intervention strategies.

·       Plan and facilitate recreational activities, day program plans, groups and workshops as assigned.

·       Research information as assigned and complete reports on the research.

·       Participate in regular team meetings as per YWCA Hamilton Developmental Services Policy.

·       Participate in training offered and attend on-going professional development opportunities as assigned

·       Implement individual program planning, personal development plans (PDP's), behavioural support plans (BSP's), protocols, and professional recommendations.

·       Implementation of effective engagement strategies, and Evidenced Based Practice, as appropriate, including and not limited to including TIP Core Practices.

·       Complete cleaning to ensure that housekeeping / cleanliness / maintenance of program areas are met

·       Ensure YWCA Hamilton workplace health and safety policies and procedures are followed at all times and act to reduce workplace hazards. Report all workplace hazards and follow YWCA Hamilton health and safety policies

·       Other duties as assigned

Qualifications

·       Minimum Education Community Participation Day Programs: Therapeutic Recreation Diploma or related Human Services Field

·       Minimum of 1 year experience working with adults diagnosed with developmental disabilities

·       Commitment to working from a feminist, anti-oppressive / anti-racist perspective

·       Experience working with individuals who are dual diagnosis

·       Experience working with individuals who may display challenging behaviours, knowledge of behavioural supports and implementation of behavioural programming

·       Experience planning and facilitating recreational activities, groups and/or workshops

·       Knowledge of Recreation Therapy Standards of Practice and therapeutic recreation philosophies

·       Knowledge of Community Resources and ability to research resources

·       Proficiency with MS Office Applications including Microsoft Word, Outlook, Excel & Publisher

·       Excellent Communication and Interpersonal Skills

·       Required to submit a self-declared medical on an annual basis

·       Possession of valid Class G Driver's License

·       Current Emergency First Aid / CPR C Certification, WHMIS, Non Violent Crisis Intervention (CPI) Certification, Transition To Independence (TIP) Certification, Quality Assurance Measures (QAM) Training, and AODA training.

NECESSARY TRAINING:

·       Non-Violent Crisis Intervention (NCI) Certification

·       Current Emergency First Aid/CPR Certification

·       WHMIS-Annually

·       Health and Safety training including 4 Step Health and Safety

·       Fire Safety/Extinguisher Training-Annually

·       QAM Training

·       AODA Training

THE FOLLOWING IS CONSIDERED AN ASSET:

·       Transition To Independence (TIP) Training

·       ARAO Training

·       Trans-Inclusion Training

·       Snoezelen Training

*Must have Valid G Driver's License and Use of a Reliable Vehicle with appropriate insurance
