WLMH Patient Flow Coordinator (PFC) works collaboratively with interdisciplinary team (including external partners/agencies) and patients/families to facilitate comprehensive discharge plans for in-patients as well as patients in the ED.  PFC is knowledgeable of destination types that will meet the needs of patients and engages the resources required to facilitate timely discharge. 

1.Undergraduate or Graduate degree in healthcare related field (Social Work, Nursing, Physiotherapy, etc)
2.Knowledge of community resources and process of referral
3. Excellent communication skills, including conflict resolution
4.Able to work independently with excellent time management skills
