About the role:

The Hospital strives to deliver care that is centred on patients and families. Patient and Family Centred Care (PFCC)

is the provision of care that is respectful of, and responsive to individual patient/family preferences, needs, and values

and ensures that those values guide all decisions. Creating a caring environment for patients and families ensures

continuity of care and fosters a learning environment. Applicants are required to have a demonstrated knowledge,

understanding and commitment to this care philosophy.

Reporting to the Manager, Indigenous Collaboration, Equity & Inclusion the Indigenous Care Coordinator (ICC) is

responsible for providing a range of health and mental health navigation, advocacy, discharge planning and supports

services to Indigenous Peoples accessing health and mental health services at TBRHSC. The ICC will work as a

liaison between the Hospital and local community organizations in order to meet the needs of Indigenous patients.

The ICC works as part of an interprofessional team, including other ICCs who report to Anishnawbe Mushkiki and

Grand Council Treaty #3.

The goal of the program is to improve equitable access to care for Indigenous patients and support their return home

through rigorous discharge planning processes and supports that leverage community/region based services that are

culturally appropriate and safe. The ICC will work with local communities to support the smooth transition of patients

back to their homes as appropriate. The ICC will be a generalist of services available for Indigenous people

throughout northwestern Ontario. The successful candidate is someone who is passionate about improving the

health and mental health outcomes of Indigenous Peoples by providing culturally appropriate and safe care.

EMPLOYMENT REQUIREMENTS:

Education/Experience:

* Current unrestricted registration as a Registered Nurse or Registered Practical Nurse with the College of Nurses of

Ontario, or unrestricted registration as Social Worker with the Ontario College of Social Workers and Social Service

Workers Master's prepared

* University degree or college diploma in health or social sciences field; work experience in the health and/or social

work field preferred

* Current certification in BCLS

* Experience working with Indigenous peoples and communities and providing culturally safe and patient centered

care

Skills/Abilities:

* Demonstrated ability to work in a collaborative team approach with corporate clients, management, medical and

support staff

* Demonstrated awareness and adherence to established standards of professional practice, personal conduct and

safety

* Experience or knowledge of the health care system, local and regional mental health and addictions resources and

availability and community resources, services and supports

* Excellent interpersonal, oral and written communication skills

* Analytical problem solving and computer skills

* Knowledge of and experience with case management

Assets:

* Knowledge of the principles of Psychosocial Rehabilitation or a similar recovery based model of care

* Fluent in Ojibway and/or Oji Cree

* Experience in discharge planning

* Experience in home care and/or community based service delivery

Salary Range: $42.870 to 53.590
