Job Responsibilities:

  • Referral management: Facilitate the referral process by reviewing requests from clinicians, assessing their appropriateness, and ensuring all required information is complete and accurate.
  • Provider collaboration: Coordinate with healthcare providers to gather additional details, clarify referral information, and ensure the timely processing of referrals for seamless care coordination.
  • Communication: Respond promptly to client and team communications, including emails, phone calls, and in-person inquiries. Triage urgent requests while maintaining professionalism and compassion in every interaction.
  • Client Liaison: Serve as the primary contact for clients, providing clear explanations of the referral process, addressing inquiries, and guiding them through next steps. Keep clients informed of their referral status and any actions required on their part.
