Position Summary
Reporting to the Team Lead, Urban Response, the Resource Navigation Nurse works in Urban Response programs, which include the Ongoing Health Services and Dynamic Overdose Response programs. These programs support clients utilizing Emergency Shelter services that are in various stages of their housing and recovery journeys. The Ongoing Health Services program provides episodic medical and social support(s) to clients experiencing homelessness and acute medical concerns while the Dynamic Overdose Response program provides emergency response and short-term medical observation to individuals who have recently experienced a medical emergency and require additional supports or intervention.
The Resource Navigation Nurse is a licensed healthcare professional that utilizes skills and knowledge to aid our clients engaged with the Medication Assistance service, within the Ongoing Health Services programming of Urban Response. This role is a part of the integrated Health & Recovery Services team and is expected to perform tasks related to medication management and teaching, applying various standardized assessment tools, symptom management, documentation, health promotion, referrals to internal and external resources, and other tasks, as assigned by a Team Lead. The Resource Navigation Nurse will work closely with Authorized Prescribers and Pharmacists to ensure that clients have all the resources required to initiate and maintain prescription medication treatments, such as income supports, regular follow up with authorized prescribers, and routine check-ins to ensure medication regimes are appropriate for the clients' care plan and/or goals.
The Resource Navigation Nurse works in a multidisciplinary team to provide client-centred, wraparound supports. The Resource Navigation Nurse will provide direct support and wayfinding assistance to these clients, with a focus on successful transition to appropriate housing options and making connections to other systems, internally and externally. The Resource Navigation Nurse will assist clients in receiving appropriate referrals to, and assessments, treatments, and services from community mental health and addictions supports and resources, internally and externally. This position will be required to accompany clients as needed to some external resources for the purpose of continuity of care, warm transfers or when advocacy is needed.
In addition to referrals, support, and wayfinding, this position may be responsible for assisting clients with obtaining necessary documentation, such as ID, or applying for required Income Support or medical benefits programs so that they may access needed treatments, such as prescription medications, or supports within the community. The position may also assist with the Intake and Discharge processes from the Medication Assistance Program offered within the Ongoing Health Services program, as well as assist clients with setting and achieving goals, especially where health and housing is a priority.
Time Commitment
40 hours per rotation with 5 days on and 2 days off. Some unusual hours may be required from time to time as the need arises. Schedule may be altered at the discretion of management.
Job Duties and Responsibilities

Apply principles of trauma-informed care in all interactions.
Support with de-escalation and crisis intervention, and overdose/emergency response, as directed.
Assist with medication administration when needed
Support with client triage, intake, assessments, and discharge processes within Urban Response programs.
Collaborate with external and internal agencies to ensure public health follow-ups are completed.
Hepatitis C Screening, Case Management and resource referrals
Work with clients towards individualized goal-setting and co-creating and updating care plans, while assisting clients in building social and/or recovery capital.

Work with clients in monitoring progress, adapting goals as needed and ultimately achieving their recovery goals.
Connect and refer clients to recovery-oriented and/or community services and programs.
Support with systems navigation (e.g. health benefits, income support).

Provide resources and referrals for ongoing client care.
Support clients in reaching goals as required, in collaboration with broader Care Team.
Share knowledge of internal and external supports and services with clients and colleagues.
Assist with special events and support external service providers within the Health & Recovery Services department.
Support with client transports to appointments or accompany individuals into the community as assigned by Team Lead.
Complete documentation in a professional and timely manner  including maintaining relevant logs and entries in the DI database.
Participate in team meetings, case conferences and ongoing professional development.
Other duties as assigned by Team Lead, or other members of Leadership team.

Required Competencies and Qualifications

Completion of an accredited two-year Practical Nursing diploma.

Registration with the College of Licensed Practical Nurses of Alberta (CLPNA). Proof of registration, in good standing with no restrictions, required.
Valid Professional Liability Insurance. Proof of coverage required.
Satisfactory Criminal Record and Vulnerable Sector Check – dated within the last 6 months. Copies required.
Current CPR – BLS. Proof of certification required.
1-2 years nursing experience is an asset, preferably within the addictions and mental health field, and/or the homeless servicing system of care.
Lived experience an asset but not a requirement. We ask that those with lived experience are two or more years in their recovery, less may be considered if ALL other aspects are met.
Experience in acute care, Advance Wound Care certification, and/or mental health experience are all considered assets but not mandatory.
Have at minimum a working understanding of Trauma Informed Care.
Strong knowledge and understanding of the epidemiology of addiction, homelessness, mental health and acute medical concerns, and able to maintain active learning on emerging evidence and best practices.
Resourcefulness and knowledge of community resources and social support systems.
Energetic and self-directed, with effective time management and organizational skills including the ability to deal with several priorities at the same time.
Well-developed interpersonal, conflict resolution and negotiating skills.
Strong problem-solving skills and the ability to improve projects, programs, and processes.
Excellent communications skills, both oral and written.
Computer skills including familiarity with word processing, database, and Internet.
