Prevention of Homelessness Case Management Worker Temporary Full Time

Overview: The Prevention of Homelessness Program assists clients in securing appropriate housing that meets their needs and provides ongoing support to ensure the client's success in their housing.The Prevention of Homelessness Case Management Worker joins a multi-faceted team working in accordance with the mission, vision and values of Sooke Shelter Society that make a difference in people's lives every day.
Job Summary: The Prevention of Homelessness Case Management Worker assists with re-housing and preventing homelessness for those recently released from incarceration, low-income individuals, and families. The Case Management Worker is expected to assist clients to maintain housing stability, by focusing on financial assistance, strength-based case management, advocacy, landlord mediation, outreach and other supportive services that result in maintaining stable, permanent housing. The Case Management Worker screens, assesses, and conducts intakes for prospective participants.
Typical Duties and Responsibilities:
- Respond to community, partner, and other inquiries and concerns respectfully and professionally as a representative of Sooke Shelter Society.
- Provide social and wellness support to a diverse adult population with complex needs. Complex issues may include substance misuse, mental health issues, neurodevelopmental disabilities, criminal justice involvement, and challenging behaviours.
- Conducts prescreens and intakes to determine participant eligibility and assess appropriate level of assistance needed.
- Assess client's needs and develop an individualized housing security plan.
- Assist individuals with identifying barriers to housing stability and engage in goal planning that aims to link individuals to the resources and skills that they need to achieve long-term housing stability.
- Engages individuals/families through assessment of strengths and needs and development of goals and service plan to ensure individuals/families remain stably housed.
- Provides and assists with information, referrals, and connections to develop a support system, including connecting clients to relevant community and government resources and services to maintain stable housing.
- Works closely with Federal and Provincial Corrections services.
- Support clients in applying for financial supports and community or government services.
- Assist clients in creating healthy life choices and positive familiar relationships.
- Build and maintain positive relationships with appropriate community resources.
- Participate in community outreach and identify and connect with potential clients.
- Assist clients in navigating interactions with landlords, neighbours and community service providers.
- Acts as an advocate in maintaining stable housing, including assistance with education on tenant's rights, landlord mediation and advocacy, and connections to legal services, as needed.
- Assists with finding and securing new housing, when needed.
- Supports with budget counseling and financial literacy services.
- Assists with immediate crisis intervention, advocacy, problem-solving and other appropriate interventions, as needed.
- Communicates with clients at regular intervals after they have received financial assistance or completed case management to assess housing status and provide additional resources or support when needed.
- Maintains case files and progress notes, adhering to professional standards and designated time frames.
- Document all pertinent client information accurately and in a timely basis in the database. Within one to three business days documented information will include client contacts, case management, advocacy, client progress, and referral appointments.
- Ensure flow of critical information through various means, including appropriate documentation, database entries, and bring forward issues and occurrences that take place on shift to management.
- Ensure all communication is up to professional standards and includes the appropriate amount of detail, while adhering to the Sooke Shelter Society's policies covering privacy and confidential information.
- Submits quarterly narrative, data, and other reports as required.
- Processes financial assistance payments, as needed.
- Provide complete case management services including file documentation and progress reports.
- Act as a first responder to overdoses and other health emergencies, administer Naloxone, CPR, and other first aid, as necessary.
- Attend and participate in staff meetings and training sessions.
- Act within the spirit and intent of the Society's policy and procedures.
- Perform other relevant job duties as assigned.
Qualifications
Education, Training, & Experience:
- Diploma in a related human/social service field.
- At least one (1) year of recent related experience, or an equivalent combination of education, training, and experience.
- Possess a Class 5 Driver's license
- Occupational First Aid Level 1 Certificate.
- Naloxone Training.
- Managing Hostile Interactions and/or Non-violent Crisis Intervention or similar training.
- Criminal Record Clearance for the Vulnerable Sector prior to commencement of employment.
- Proof of COVID-19 immunization is a requirement.
Skills/Abilities:
- Basic computer skills with a working knowledge of Microsoft 365.
- Demonstrated knowledge/familiarity with related resources such as mental health systems and related agencies, welfare system, and addiction support agencies.
· Understanding of issues related to mental health, trauma, and addictions.
· Demonstrated ability to respect all clients and treat them with dignity while maintaining professional boundaries.
· Demonstrated ability to use sound judgement while working in demanding conditions and circumstances.
· Demonstrated suitability to work with adults facing multiple barriers and presenting challenging behaviours in a diverse environment.

- Demonstrated physical ability to perform the duties of the job.

- Demonstrated ability to apply and follow safe work procedures.
- Physically capable of completing duties outlined in first aid training including kneeling, placing body in recovery position, administering Naloxone, and giving breaths/compressions as required.
These positions require union membership.

Pay: $27.54-$31.61 per hour
