Are you a person who is a self-starter, with a passion for working with a growing non-profit organization that supports First Nations, Inuit, and Metis peoples experiencing homelessness? The Aboriginal Coalition to End Homelessness Society (ACEH) is seeking a Housing Transitions Support Worker for a 4-month pilot project focused on walking alongside Family Members who are ready to move from culturally supportive housing into independent market housing.

We are a non-profit, registered charity and culturally supportive housing provider dedicated to supporting First Nations, Inuit, and Metis peoples experiencing or at risk of homelessness. Our work is Indigenous led and governed, and centers around the voices of our Indigenous Street Community, whom we refer to as our Family Members. Through building partnerships locally and across the Island, we identify and address systemic and structural barriers, share information, resources, and lessons learned, and provide Indigenized harm reduction, cultural support services, and housing.

Our Vision: Our way is to care for all our people, from the youngest to the oldest. We are all one. Some of our people living away from home are suffering, isolated, and homeless. We stand together to end homelessness.

Job Details:

Roles and Responsibilities

This role supports Family Members who are preparing to transition into independent housing but may need short-term, focused, one-on-one support to build stability, confidence, life skills, and community connections.

Each worker supports up to three Family Members, ensuring meaningful, consistent, and culturally safe support. This role bridges a crucial gap between supportive housing and long-term independence as well as include hands-on support with goal planning, life skills, tenancy readiness, appointments, moving logistics, and cultural connections.

Relationship-Building & Support

  • Build trusting, caring, and reciprocal relationships with 2–3 Family Members selected for the pilot.
  • Develop individualized goal plans and housing transition plans.
  • Celebrate milestones and encourage growth through strength-based practice.
  • Support development of independent life skills (budgeting, cleaning, cooking, grocery shopping, organization, utilities, banking, bill payments, tenancy expectations, etc.).

Housing Transitions

  • Support securing suitable rental units in collaboration with CAIS and House Managers.
  • Assist with moving Family Members: packing, transport, set-up, purchasing basic home items.
  • Help with IDs (renewals/applications), income assistance, CAIS rental supplement applications, and required forms.

Cultural & Community Connections

  • Support participation in cultural programming, ceremonies, home-community visits, and land-based healing.
  • Help connect Family Members to neighbourhood resources, health care, workshops, and other community supports.

Case Management & Documentation

  • Participate in case management with CAIS and House Managers.
  • Track progress, update transition plans, and adjust supports as needed.
  • Maintain accurate documentation in CAMS; track activities, outcomes, and pilot data.

Operational & Program Support

  • Manage program supplies and basic-needs budget.
  • Provide advocacy at appointments (e.g. health, mental health, addiction supports, landlord meetings).
  • Assist with problem-solving, conflict resolution, and non-crisis de-escalation.

  • Contribute lessons learned and recommendations at the end of the pilot.

Qualifications and Educational Experiences:

  • Experience and/or personal insights into working cooperatively and non-judgmentally with people with disabilities, lived experiences, and addictions.

  • Experience working with First Nations, Inuit, and Metis peoples and marginalized populations.

  • Experience with non-profit organizations, family support, social services, and/or chronic homelessness is an asset.

  • Experience in coordinating events and community engagement activities.

  • Current First Aid and CPR certifications are preferred.

  • Possession of a valid Class 5 Driver's License and access to a reliable car is as asset.

Knowledge, Skills, and Abilities:

  • Progressive understanding of persistent mental illness, addictions, and other challenges impacting overall health and well-being.

  • Ability to work holistically, professionally, and non-judgmentally with diverse personalities, needs, cultures, lived experiences, and abilities.

  • Ability to effectively respond and adjust to unexpected events, assess high-risk situations, problem-solve, provide crisis intervention, and manage conflict in a culturally supportive manner.

  • Ability to work independently and manage competing demands while adhering to deadlines in a fast-paced environment.

  • Effective written and verbal communication skills to facilitate collaborative practice, culturally safe environments, and a respectful workplace.

  • Demonstrated understanding of intergenerational trauma from colonization, Residential Schools, the Sixties Scoop.
