Job Description
Position: Recovery Caseworker
Reports To: Manager of Recovery
Department: Recovery
Resident-Focused Duties
- Develop individualized case plans and set measurable goals in partnership with each resident.
- Conduct regular one-on-one meetings to monitor progress, address challenges, and update care plans as needed.
- Coordinate and, when necessary, attend resident appointments with medical, legal, and professional service providers.
- Support residents in rebuilding healthy family relationships and, where appropriate, assist with reunification efforts in collaboration with relevant agencies or community supports.
- Assist residents in preparing résumés, applying for employment, and accessing educational or vocational training opportunities.
- Support residents in establishing essential financial accounts and ensuring income tax and government documentation are current.
- Assist residents in setting up bank accounts and developing basic budgeting and money-management skills.
- Support residents in preparing for transition from the program, including securing housing and developing independent living skills.
- Support residents in obtaining YMCA memberships or other healthy-living resources to encourage physical wellness.
- Encourage personal growth and spiritual development as part of holistic recovery.
Casework Responsibilities
- Conduct comprehensive assessments (e.g., SPDAT) within 30 days of admission and ensure personalized action plans are completed within 60 days.
- Ensure that all care plans are implemented, regularly reviewed, and producing measurable results.
- Coordinate with internal and external service providers, including healthcare, mental-health, and social-service agencies, to meet resident needs.
- Ensure each resident is connected to appropriate external or community-based supports, such as mentors, counsellors, or recovery networks, before transitioning from the program.
- Provide ongoing education to residents on relapse prevention, community resources, and healthy-living strategies.
- Promote cost-effective, high-quality interventions that align with organizational goals and resident-care objectives.
- Apply evidence-informed approaches and best practices to case planning and resident support to ensure consistent, high-quality service delivery.
- Participate in phase reviews and progress evaluations in collaboration with the Recovery Supervisor and Manager of Recovery.
Administrative Responsibilities
- Maintain accurate and timely documentation of resident progress, phase plans, and weekly updates in HIFIS and electronic files.
- Prepare residents for successful transition and program completion through discharge planning and coordination with community supports.
- Collaborate with the Recovery Supervisor to ensure communication between care planning and daily operations remains consistent.
- Coordinate with volunteers, teachers, and employment partners to support Phase 2 residents in work readiness and skill development.
- Complete required reports, updates, and correspondence as directed by the Manager of Recovery.

Education and Experience
- Post-secondary education in a related field or experience in addictions recovery, mental health, or human services is considered an asset.
Mandatory Skills and Certifications
- First Aid / CPR-C
- Criminal Record Check / Vulnerable Sector Screening
- ASIST (Applied Suicide Intervention Skills Training)
- NVCI (Non-Violent Crisis Intervention)
- Basic computer proficiency, including Office 365 and HIFIS.
Schedule
Day shift, Monday to Friday, 8:30 a.m. – 4:30 p.m.
Job Types: Full-time, Permanent
Pay: $42,750.00-$45,760.00 per year
