T- FT/PT Integrated Care Coordinator Trauma-Informed Youth Services

HealthOPM is recruiting on behalf of a respected Toronto-based non-profit organization that operates a trauma-informed, anti-human trafficking (AHT) live-in treatment program for high-needs youth The Integrated Care Coordinator provides intake, assessment, complex care navigation, individual and group-based clinical support, and evidence-based treatment groups to youth ages 12-17 who are survivors of sex trafficking and/or sexual exploitation, within an Anti-Human Trafficking live-in treatment program. This program offers 24/7 supervision, support, and care for youth 365 days per year. This opportunity is with a Toronto-based non-profit organization that has developed a specialized residential program for female youth ages 1217 recovering from sexual exploitation and trafficking.
Salary range:- $63,071 to $73,447 per annum (Full-time)
- $30.32- $35.31 per hour (Part-time)

Location: North York, ON.
WHAT YOU WILL DO- Connect with the child/youth, referring support person and/or family to obtain information, complete intake assessments and determine eligibility for the AHT Live-in Treatment Program.
- Collaborate with the interdisciplinary team to assess and address clients need and determine action plans with clients to address risk and safety concerns.
- Provide clinical support and treatment that is evidence-informed and aligns with client goals and objectives. Support client mental health and wellbeing within the live-in treatment environment.
- Facilitate complex care navigation, as well as weekly individual and group therapeutic treatment sessions for clients, including documentation and administrative tasks to support client needs and ensure compliance with Ministry and Licensing body requirements.
- Ensure clients have flexible and accessible pathways to engage in treatment components of program.
- Knowledge of the ICYMH sector, including Anti-Human Trafficking programs and service providers in the GTA, to ensure excellence in service navigation for children, youth, and families by making referrals to appropriate programs.
- Ensure accurate and timely documentation in TREAT in accordance with guidelines, agency, and accreditation standards.
The following education, professional memberships and knowledge are a requirement for this role:
- Bachelor's degree in social work or other Health-Related discipline is required.
- A master's degree in social work or psychology or in a Health-Related discipline is preferred.
- Minimum of 3 years of related experience.
- Registered in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) or with the College of Registered Psychotherapists of Ontario (CRPO).
- Experience in crisis management, and trauma informed care principles and practices.
- Experience providing a broad range of clinical services and demonstrated effectiveness working with sexually exploited youth.
- Self-awareness, ability to set boundaries and engage in self-care practices.
- Awareness of the role of inclusion, diversity, equity, and accessibility (IDEA) in the workplace and within the community being served by the position.
- An understanding of anti-black racism, anti-oppressive/anti-racism perspective in the context of the social determinants of health.
- Ability to recognize and identify situations where incidents of imbalance of power and privilege may occur and can result in possible negative impacts.
While not required, preference will be given to candidates with the following:
- Functional computer and client information system knowledge.
- Fluency in another language(s), including sign language.
