The Enhanced Housing Support Program provides intensive one-to-one practical support and client-centered case management to newly housed Black and LGBTQ2S+ adults, seniors, and individuals dealing with mental health & addiction challenges who have experienced long-term homelessness to provide housing stabilization, connect to community based- support and services. In partnership with City of Toronto -Housing focused client portfolio. The Specialized Housing Case Managers receive referrals from the City of Toronto's Coordinate Access and PATH.
The Relief Specialized Housing Case Manager works as part of a team that perform most of their work offsite and independently, in community agencies and tenant apartments. Specialized Housing Case Managers provide Critical Time Intervention (CTI) case management services – a time-limited evidence-based practice that mobilizes support for most vulnerable individuals during periods of transition, The case management model will focus on facilitating community integration and continuity of care by ensuring that clients develop linkages in their community which enhances housing stabilization.
The ideal candidate will have extensive experience working with people who are homeless, risk of homelessness or inadequately housed people who may have complex and chronic substance use, mental or physical health issues, and involvement in the justice system, trauma, social isolation, immigration, and unemployment or underemployment.

What You Will Do:
Ensure clients receive effective housing stabilization support as outlined in the Housing First Model by the City of Toronto.
Provide high quality, client centered case management utilizing evidence-based practices of motivational interviewing, trauma-informed care and harm reduction. Using creativity and perseverance when necessary to engage the most difficult to engage residents with close monitoring of the program manager.
Provide follow up services to address the budgeting, adjustment, physical health, emotional health, and employment issues of the Enhanced housing support Program clients.
Provide time limited housing support and case management, including assessment, referral, education, mediation and/or negotiation with landlords or other tenants.
Maintain regular contact with clients through home visits, telephone contact and written communication.
Ensure accurate records by way of thorough outcome-based recording of client encounters.
Provide crisis intervention for clients facing distressful situations by collaborating with other professionals.
Support to individuals with complex mental health, addiction, and physical health needs, who have been housed by city of Toronto partner agency.
Develop and implement plans of care with short and long-term objectives, with ongoing monitoring.
Collaborates with the City of Toronto, judicial, health and other systems, organizations, and individuals to ensure a well-coordinated service system. What You Bring to the Team:
•
A Degree or College Diploma in a related field.
- At least two years of experience working in community settings with individuals and households exiting homelessness to work towards continued housing stability. And living with mental health and substance use challenges.
•
Strong commitment to learning and using Harm Reduction, Motivational Interviewing and Strengths based skills in order to promote improved behavioral health among clients.
•
Demonstrated outreach experience working with Black adults or seniors who have experienced homelessness, mental health, and/or addiction issues.
•
Excellent knowledge of community resources specific to the homeless and/or demonstrated ability to search for applicable resources/information.
•
Excellent interpersonal skills, strong ability to form relationships with marginalized individuals.
- Basic competency of Microsoft Office.

Compensation and Benefits:
For the Relief Specialized Housing Case Manager, Fred Victor offers an hourly rate of $22.65
