Job Overview
Under the authority of the Head of Outpatient Services, the clinical nurse and case manager in outpatient consultations is responsible for coordinating the care and activities of patients in a specific population from admission to discharge.
She applies her knowledge of the FOCUSED philosophy: family- and patient-centered care, open communication, collaboration, understanding and compassion, safety and transparency, expertise and education, motivated by research and best practices.
Responsibilities
• Reviews patient medical records prior to clinic and ensures that all orders, diagnostic tests, and referrals are complete and results are available. Contacts family as needed.
• Ensures that the initial intake/visit is coordinated and effective in developing a care plan for patients.
• Responds to care needs and refers patients to available resources and support systems to meet those needs, i.e., children with special health care needs, early intervention programs, academic and vocational rehabilitation.
• Works collaboratively with the multidisciplinary team and shares responsibility for patient care.
• Communicates regularly with the inpatient care team and clinical team to ensure timely care and services, identify needs, and develop an action plan as needed.
• Documents care/service coordination activities clearly, concisely, and in accordance with hospital, Accreditation Canada, and OIIQ policies and procedures. Ensures that all required regulatory documentation is present in the file.
• Provides comprehensive education to patients and their families in the clinic.
• Strives for continuous improvement based on best practices, survey results, complaints, and any other improvement initiatives from members of the healthcare team.
Qualifications
Academic qualifications
• Bachelor's degree in nursing
• Member in good standing of the OIIQ
Experience
• Two (2) years of experience in acute care
• One (1) year of experience in pediatrics, an asset
• One (1) year of experience in orthopedics, an asset
Required skills
• Fluency in written and spoken English and French
• Knowledge of a third language, an asset
• Good judgment to correctly prioritize work tasks
• Good organizational skills
• Autonomy, initiative, and punctuality
