About the role:

The Hospital strives to deliver care that is centred on patients and families. Patient and Family Centred Care (PFCC)

is the provision of care that is respectful of, and responsive to individual patient/family preferences, needs, and values

and ensures that those values guide all decisions. Creating a caring environment for patients and families ensures

continuity of care and fosters a learning environment. Applicants are required to have a demonstrated knowledge,

understanding and commitment to this care philosophy.

Reporting to the Manager ‑ Complex & Chronic Diseases, with a matrix reporting relationship to the Chief Nursing

Officer, the Nurse Practitioner provides services for patients experiencing both acute and complex chronic disease,

with a key focus on diabetic care

Applicants require the following qualifications:

*Current registration as a Registered Nurse Extended Class with the College of Nurses of Ontario

* Successful completion of Nurse Practitioner program

*Current certification in Basic Cardiac Life Support

*Minimum of 3‑5 years recent clinical experience with patients with complicated case histories associated with

complex chronic disease, including diabetes care, preferably in the acute care setting

*Applicants must be eligible to write the Canadian Diabetes Certification Board exam within one year of start date

*Minimum of 2 years recent experience working in an inter‑professional team environment

*Minimum of 2 years recent experience in teaching and counseling

*Preference will be given to those currently certified as a Certified Diabetes Educator

*Experience in a Nurse Practitioner role is an asset

*Completion of College Diabetes Education Certificate is an asset

*Proficient in computer applications and software such as Outlook, Meditech and Community Wide Scheduling,

Microsoft Word, PowerPoint and Excel

In addition, applicants will have skill and experience in the following elements of nursing practice:

*Physical Assessment and ability to develop, implement and evaluate comprehensive, interdisciplinary teaching

programs, care plans and follow‑up for Complex Chronic patients

*Effective patient and family counseling skills

* Ability to work effectively both individually and as part of a team

*Strong organizational, critical appraisal and analytical skills

*Excellent interpersonal/communication skills

*Proficient in computer applications and software such as GroupWise, Microsoft Word, PowerPoint and Excel

*An understanding of healthcare issues within the community and experience in health services research, and

program evaluation

*Demonstrates coaching and mentoring skills

*Knowledge and experience in change theory, adult learning principles and health promotion, Self Management of

Chronic Diseases

*High level of initiative and self direction

*The incumbent will be expected to complete the credentialing process in conjunction with the standards of the

Department of Medical and Academic Affairs

In addition to the above qualifications, preference will be given to applicants who:

Salary Range: $67.170 to 74.460
