The Squamish Nation's Yúustway Health & Wellness Department is seeking a Manager, Home and Community Care (HCC) to lead a multidisciplinary team that provides holistic, culturally safe care to Elders, individuals, and families across the Nation.

This senior role is ideal for a Registered Nurse who believes in relational leadership, operational excellence, and supporting our people with dignity, respect, and lateral kindness. You will guide a team of ~30 care professionals who deliver essential home care, case management, daily living support, and Better at Home services to Nation Members from birth to end of life.

What You'll Do
  • Oversee care delivery for Squamish Nation Members in North/West Vancouver and the Squamish Valley.
  • Ensure care plans are implemented following cultural safety, humility, and trauma-informed best practices.
  • Promote a therapeutic, respectful environment rooted in Skwx̱wú7mesh Úxwumixw values.
  • Supervise Team Lead RN, Occupational Therapist, and Better at Home Coordinator.
  • Lead a dedicated team of Nurse Assessors, Client Care Coordinators, Health Care Assistants, Homemakers, and contracted partners.
  • Support recruitment, onboarding, performance evaluations, staff development, and lateral kindness.
  • Manage annual work plans, scheduling, EMR documentation, reporting, and compliance.
  • Oversee budgets, equipment loan programs, and coordination with Finance and Health Benefits.
  • Lead quality improvement initiatives and support policy, procedure, and service design.
  • Collaborate with Yúustway leadership, Elders, allied health partners, Squamish Nation departments, FNHA, VCH, ISC, United Way BC, and community partners.
  • Oversee the Better at Home Program including reporting, volunteer coordination, and funder relationships.
  • Ensure staff adhere to health and safety standards, confidentiality, respectful workplace policies, and professional practice guidelines (BCCNM).
  • Manage responses to adult protection concerns and support risk mitigation.
What We're Looking For
  • Registered Nurse with current BCCNM practicing registration.
  • Relevant experience in home care, community care, or health care leadership.
  • Experience managing multidisciplinary health teams.
  • Knowledge of delegated/assigned tasks, provincial HCC regulations, and community-based care models.
  • Experience working in First Nations communities or a strong commitment to Indigenous Cultural Safety.
  • Skilled in case management, care coordination, trauma-informed care, and quality improvement.
  • Strong communication, diplomacy, time management, and organizational skills.
  • Emotional intelligence, cultural humility, and a collaborative leadership style.
  • Ability to maintain calm, deliver difficult information respectfully, and build trust.
  • Confident using EMR systems and Microsoft Office.
  • Cultural awareness and deep respect for Sḵwx̱wú7mesh Úxwumixw values, language, and traditions.
  • A valid BC Driver's License Class 4 or 5 Class

Salary: $94,000 - $141,000

Candidates are typically offered compensation between the beginning to the midpoint of the range. We are committed to providing a fair and equitable package that reflects the value an individual brings to the organization.
