Care Manager
Do people naturally open up to you? Have you been told that you bring calm to chaos? Are you the type of person who shows up fully present for others, not because it’s your job, but because it’s who you are?
You’re a people first kind of nurse. You see the whole human, not just their diagnosis, and you’re guided by the belief that dignity, joy, and deep human connection should be part of every care plan. Your clinical training gives you confidence, but it’s your kindness, empathy, and quiet leadership that make people trust you.
At Hero Home Care, we believe that there is an art and a science to caring and our core purpose is “Happy Humans and Healthier Communities”. That’s not just a slogan. It’s a call to action and we hope you are the kind of person who answers that call with both heart and hustle. You are proud to do hard things for the right reasons. You don’t shy away from complicated situations or tough conversations. You show up, you listen, and you lead with compassion.
You’re the kind of nurse who sees the details others might miss. You care enough to follow through, to make one more phone call, or to take the time to truly understand a family’s needs. You like structure and systems, but you’re also flexible. You know that great care isn’t always predictable, and you’re okay with that, so you always go above and beyond. Whether you’re building a care plan, supporting a caregiver (we call them heroes), or helping a family navigate change, you’re steady, warm, and intentional. You believe that how we do things matters. You want to create more moments that matter and you want to make them count
A Day In the Life:
There may be no such thing as a “typical day” in the life of a Care Manager, but if there were, it might begin like this:
You’re sitting at your kitchen table, coffee or tea in hand, the early light creeping in through the window. You take a moment to scan your emails, check in with the team, and review the day ahead. Your schedule is full, but your purpose is clear. You’re here to create more moments that matter for families facing one of the most important decisions they’ll ever make.
Your first visit of the day is with a family considering care for their mother, who’s recently had a fall. They’re worried and overwhelmed, but that’s where you shine. You arrive not just with expertise, but with calm. You listen deeply, noticing what’s said and unsaid. You ask thoughtful questions, read the room, and create space for emotion.
You offer solutions tailored to the client’s unique needs, rooted in compassion, experience, and a deep understanding of what quality care truly looks like. You bring clarity where there’s confusion, confidence where there’s doubt, and peace of mind to a concerned daughter who hasn’t slept well in weeks.
Mid-morning, you pull over safely and sign in to the company-wide daily huddle. It’s a quick seven-minute touchpoint where the whole team connects to share good news, celebrate milestones, recognize our Heroes, and get aligned on the top priorities for the day. It’s a small moment that keeps the entire organization feeling close, energized, and moving in the same direction.
For you, nursing is not just a profession. It’s a calling. A way of showing up in the world.
Throughout the day, you might connect with our Care Hub about scheduling, follow up on a new client’s care plan, or touch base with a Hero in the field to offer support and guidance. You balance clinical oversight with coaching and mentorship, helping our team deliver on the promise of exceptional care. You’re not just leading. You’re lifting others.
Behind you is a team that shares your passion. At Hero, we support each other, challenge each other, and hold each other to a high standard, always grounded in our core values (see below).
This isn’t easy work. But it’s meaningful, rewarding, and deeply human. And you wouldn’t have it any other way.
Our Core Values:
- People Come First: We don’t just recruit caregivers, we hire heroes. Because we know it’s people that make the home care business tick. We put people first, our team members and our customers. We treat all people equally and celebrate diversity. We communicate openly and honestly. And when it comes to our team members or customer’s health and happiness, we keep the focus clear.
- Commit To It: We’re in the business of caring for others, so that means we’re in the business of making quality decisions and doing whatever it takes to fulfill our purpose of creating happy humans and healthy communities. We do what’s right, not what’s easy. We also appreciate that at the community level, we’re only as good as our word. We see things through and walk our heroic talk.
- Go Above & Beyond: Our heroes know they will be called upon to deliver above and beyond. But it’s not just about energy – which we have a lot of – it’s about inspiration. We think creatively and bring joy to the work because we love serving others and the community. Some days it’s about preparing a delicious meal – other days it’s about replacing the elastic waistband in a customer’s favorite pajamas (true story).
- We Do 1% Better: In life, there are rarely silver bullets and one-size-fits-all solutions. So instead, we commit to the journey of continuous improvement. Lasting change through small improvements one day at a time. We want to lead our industry because our industry needs a hero. We are open to coaching and receiving feedback because when we grow our customers and communities do better.
If this feels like the kind of team you’ve been looking for, we’d love to meet you.

- Liaising with your Care Support team to communicate any changes to care and ensuring that customer coverage is secured and assisting with scheduling as needed.
