Visit us today and check out to see why more people from across the globe are moving here.About The OpportunityThe Care Coordinator is responsible for the assessment, care planning, authorization of services and the ongoing case management of clients referred to Continuing Care. Care coordinators help provide an array of services/programs offered by Nova Scotia Health Continuing Care and Department of Seniors and Long Term Care to assist individuals and families cope with complicated acute or chronic health situations in the most effective way possible in the community. Care Coordinators assist clients to identify their goals, unmet needs and resource requirements. Based on the assessment, the Care Coordinator assists the client and their care partners to formulate a plan to meet identified goals.The Care Coordinator authorizes services and makes referrals/linkages to other professional and volunteer resources, to assess and support community-dwelling clients for as long as possible. They work with interprofessional teams to facilitate hospital discharge to return to the community.Care Coordinators work in the community, hospital or long term care facilities. They are fiscally responsible for the care, services, and equipment they authorize within available Department of Seniors and Long Term Care policies and programs. In exceptional circumstances, Care Coordinators work with clients to articulate and advocate for alternative solutions to address unmet needs. They maintain ongoing communication with clients, family, caregivers, care providers and health care teams within the client's identified circle of care. The philosophy of the Care Coordinators is grounded in "Home First": maximizing available services and resources to support clients and families in the home/community. They are responsible for reassessments at routine intervals, and skilled care management.This specific opportunity is to work as part of the new interdisciplinary Seniors Care Team providing services in the Dartmouth General Hospital's emergency department and community setting throughout Central Zone to seniors > 65 years of age. Under the supervision of a CZ Manager, the Care Coordinator provides collaborative, client-centered care, supporting the physical, psychosocial and emotional well-being of all individuals (including clients, families, groups, populations and communities). The Care Coordinator participates in the triage, assessment, planning, delivery and transfer of home support services that will support seniors to enhance their independence and function at home. The position requires the ability to work independently and collaboratively to the full scope of their practice within a busy multidisciplinary environment. The Care Coordinator must have the ability to shift focus and attention to prioritize workload and manage time efficiently. Strong interpersonal and written communication skills are required. Experience working in a community setting is an asset.About YouWe would love to hear from you if you have the following:Bachelor's Degree in Nursing, Social Work, Occupational Therapy, Physiotherapy or Nutritional StudiesCurrent and ongoing registration (or eligibility for registration) with the applicable Nova Scotia Professional Regulatory bodyMinimum three (3) years' experience with the professional regulatory body- Preference given to those individuals with more than 5 years' experience and those who have experience working in the community.Demonstrated knowledge and/or experience in a community or long-term care setting requiredExperience in Geriatric Care preferredKnowledge and/or experience in dedicated case management or discharge planningCompetent with computer use, keyboarding and be able to conduct assessments using a laptopKnowledge of community agencies and resourcesExcellent assessment, interviewing and negotiation skillsStrong verbal and written communication skillsAbility to work independently and co-operatively in a busy multidisciplinary team contextAbility to multi-task, prioritize workload and manage time efficientlyValid driver's license and access to a reliable vehicle are requiredCompetencies in other languages preferred, French an assetFirst Aid/CPRTravel to visit clients in the place they call home within Central Zone is requiredExperience working in a team-based care setting is considered an assetPlease ensure your resume is up to date and includes all relevant education, experience, training, and certifications.*Hours*Permanent Full-Time / 75 hours bi-weekly / weekend shifts (approximately one per month)Shifts may also include evenings and holidaysCompensation And Benefits$ $55.57 HourlySuccessful candidates may be eligible for our benefits package which includes health, dental, travel, long-term disability, and life insurance coverage as well as a defined benefit pension plan.Once You've AppliedThank you for your interest in this position.
