The Government of Yukon's Continuing Care division is known for its exceptional standards, providing clients with the highest level of care and a healthy workplace for staff. Continuing Care's mission is to provide comprehensive care and services, including long-term care, home care, and regional therapy services, to the people of the Yukon. The Yukon Home Care Program provides client-focused health services in the home, improving quality of life and supporting Yukoners to live independently.
Your role
Home Care is seeking a dynamic leader with experience in home and community care and demonstrated supervisory and leadership skills. A deep understanding of and commitment to cultural humility and serving the unique needs of rural Yukon communities is essential.
Reporting to the Manager, Home Care, this position provides and serving clinical supervision for an interdisciplinary team operating throughout the territory. The successful candidate will have proven ability to work collaboratively and respectfully alongside First Nation governments and their citizens, community partners, and other Health and Social Services branches. This position is critical to supporting the growth of Home Care in our rural communities, ensuring client centered service delivery and fostering a supportive environment for staff.

Essential qualifications
Please submit your resume clearly demonstrating how you meet the qualifications below. Selections for further consideration will be based solely on the information you provide in your resume.
- Current registration as a health care professional in one of the following disciplines: Occupational Therapist, Physiotherapist, Registered Nurse, LPN or Social Worker
- Experience planning, developing, implementing and evaluating quality improvement initiatives
- Experience working as part of an interdisciplinary healthcare team
- Experience supervising employees
- Experience establishing and maintaining respectful collaborative partnerships with First Nation governments and/or Indigenous communities would be considered an asset

Desired knowledge, skills and suitability

- Exceptional leadership skills with an ability to develop, mentor and coach staff fostering a culture of continuous learning and supporting the facilitation of knowledge transfer
- Ability to lead the delivery of clinical services in a community-based setting
- Demonstrated self-awareness and professionalism, including high levels of emotional intelligence, integrity and ethical practice
- Strong analytical and problem-solving ability to identify, analyze, develop and implement effective solutions for staff and service issues, problems and operational challenges
- Ability to lead, manager and sustain quality improvement initiatives
- Advanced knowledge and clinical experience in care coordination
- Highly effective communication skills (both oral and written)
- Ability to build and maintain effective working relationships and professional partnerships with internal and external stakeholders
- Strong organizational skills and time management skills to support a diverse territorial team and competing priorities
- Ability to take a broad-based, strategic approach to develop and adapt new or changing services to effectively meet the evolving needs and diverse circumstances of clients
- Commitment to providing client centered quality client care
- Ability to work in a multicultural environment, demonstrating a high level of cultural humility, while demonstrating a dedication to fostering cultural safety

Conditions of employment
You must meet the following conditions before being hired, or within the time frame noted.
- TB Screening
- RCMP Vulnerable Sector Check
- CPR-C with AED (completed within last 30 months)
- WHMIS 2015
- Valid Class 5 Driver's License with no restrictions
- Current registration with a health professional regulatory body
Job requirements
This job requires you to perform, and/or be exposed to, the following:
- Exposure to animals
- Exposure to second-hand smoke from clients
- Exposure to latex products
- Heavy lifting and bending
- Working in rural communities; traveling by both road and air, in and out of the Yukon Territory is a requirement. This position is also required to provide clinical coverage in rural communities as needed.
