About the Role:
The Lung Screening Program Specialist works as part of the interdisciplinary health care team to support the delivery of the Ontario Lung Screening Program (OLSP). The Lung Screening Program Specialist provides patient assessment, coordination, education, and follow-up services for individuals at high risk of lung cancer undergoing low-dose computed tomography (LDCT) screening.
The role supports patients through the screening pathway by coordinating appointments, communicating results and next steps, providing informed consent education, and offering smoking cessation counselling and resource support. In collaboration with the interprofessional team, the Lung Screening Program Specialist ensures timely follow-up, continuity of care, and delivery of high-quality, patient-centered screening services in accordance with OLSP standards.

What you'll be doing:
Conduct OLSP risk triage and comprehensive eligibility assessments using standardized tools to confirm inclusion and exclusion criteria and determine screening eligibility.
Collect, assess, and document relevant clinical and risk information, including age, smoking history, and additional risk factors such as COPD and family history of lung cancer.
Coordinate care for patients referred to OLSP, including scheduling of risk assessments, LDCT scans, smoking cessation counselling, and other referrals as appropriate .
Coordinate post-LDCT follow-up, recall, and referrals in accordance with OLSP guidelines and Lung-RADS management recommendations.
Communicate screening eligibility, participation status, results, and next steps to participants and referring providers in a timely and accurate manner.
Ensure continuity of care within the screening program by coordinating ongoing participation, including booking subsequent LDCT appointments.
Facilitate connection to a provider for eligible, unattached participants who choose to proceed with screening.
Provide informed consent education and patient- centred counselling regarding LDCT screening, results interpretation, and next steps.
Deliver smoking cessation counselling and facilitate access to appropriate cessation resources in accordance with clinical practice guidelines.
What you bring:
Undergraduate degree in a health-related field (e.g., Bachelor of Science, Medical Laboratory Technology, or equivalent).
Minimum three (3) years of relevant experience in patient care coordination, discharge planning, or related clinical practice within an acute care setting (preferred) Clinical knowledge, patient education skills, and care coordination expertise acquired through formal education and professional experience.
Knowledge of office routines, healthcare documentation standards, and administrative procedures.
Ability to use specialized healthcare systems, screening tools, and diagnostic technologies.
Good u nderstanding of clinical pathways, screening protocols, and multidisciplinary care coordination.
Strong communication and counselling skills to support informed decision-making and patient education.
Ability to work independently while collaborating effectively within an interdisciplinary health care team.
Demonstrate s strong communication and interpersonal skills, with the ability to work collaboratively with patients/families, interprofessional team members, and other stakeholders
