As we expand our services, we are seeking a highly skilled and compassionate Nurse Practitioner to join our team.

Responsibilities include:

  • Provide a comprehensive health assessment on patients which involves completing a health history and physical examination, ordering and interpreting laboratory or diagnostic tests, diagnosing medical conditions, initiating or adjusting treatments, monitor patient'scondition, prescribing and administering medications, and education/counselling patients and their families in current, preventative and follow up care. Makes appropriate referral to other health care providers, as necessary and appropriate based on patient needs and requirements of care.
  • Reviews, interprets, and communicates patient results of investigations and diagnostic tests to patients, their families, and their primary providers when appropriate.
  • Collaborates with other members of the health care team and specialist(s) involved in their patients care when necessary.
  • Identifies learning needs of patients, families, nurses and others involved to provide education on treatments/conditions.
  • Provide support and counselling to patients and their families for proper health promotion and disease prevention.
  • Provide educational opportunities to students through mentorship and preceptorship.
  • Documents all patient information including discharge summaries, clinic reports, examinations, plans of care, treatments and follow up care.
  • Identifies, develops, plans, coordinates, implements, and evaluates health care plans/health promotion/disease prevention/chronic disease management/treatments/follow up care with patients and their families.
  • Keeps up to date on current practices and guidelines, incorporates current research into practice and current practice guidelines.
  • Participates in continuing educational opportunities to enhance nursing practice and current guidelines in patient care and treatments.
  • Participate in clinical audits, incident reviews, and implementation of quality improvements under the Medical Director.
  • Provide preceptorship for students and support RN Prescribers or new NPs.
  • Participate in multidisciplinary rounds, case review meetings, and data collection supporting KPI tracking.

Qualifications:

  • Completion of a university, college or other approved registered nursing and nurse practitioner program is required.
  • A master's or doctoral degree in nursing is required for clinical nurse specialists.
  • Minimum 2-3 years' experience in primary health care/family of all ages
  • Current registration in good standing with the appropriate regulatory college for the jurisdiction of practice.
  • Continuous education and training of advanced nurse practitioner skills which may include recertification (ie. ACLS, SANE, Wound and Diabetes Care, Joint Injections, etc.)
  • Demonstrated ability to work independently and as a team member in a changing and complex interdisciplinary environment.
  • Demonstrated strong problem solving, analytical and critical thinking skills.
  • Excellent communication and facilitation skills and the ability to foster teamwork and collaboration.
  • Consistent demonstration of Catalyst Health Solution's corporate values.
  • First Aid, CPR, and BLS and any required certificates associated with the position. ACLS encouraged.

This job position will be online, work from home. Starting wage is $55/hour. We are hiring 1 full-time, 1 part-time and 3 casual positions. Option to work as an employee or as an independent contractor.
