Our Social Prescribing Program works alongside clinical treatments to address senior’s needs in a holistic and inclusive way. Social Prescription can include physical activities, social activities, nutrition/food security programs, caregiver programs, and opportunities to access services in the community. Social Prescribing at Community Services provides support by identifying what is required to improve overall well-being of seniors in our communities.
RESPONSIBILITIES:
- Work in collaboration health care referrers and other Seniors Community Connectors in the province to create an environment where seniors at-risk of frailty will be able to access resources and support.
- Conduct home visits with seniors to do the intake and discuss what resources and activities they feel would benefit them, create health and wellness plans and regularly monitor these plans with the client.
- Provide referral sources, where applicable, 6-month updates on how the senior is doing.
- Participate in Community of Practice meetings with other Social Prescribing programs on a regular and consistent basis.
- Work with other program staff in the Seniors Department, in organizing and hosting volunteer dropins and volunteer appreciation events.
- Participate in United Way meetings and conferences, as requested.
- Facilitate presentations to a variety of stakeholders, including but not limited to, physicians, nurse practitioners, and the United Way.
- Meeting regularly with the Program Director for regular supervision meetings, budget review, program status update and informing the Program Director about issues and concerns.
- Represent and promote the Social Prescribing Program and the Agency at community meetings, as requested by the Program Supervisor.
- Maintenance of program records, in accordance with the funder Letter of Agreement and
accreditation standards.
- Become an active member of one or more Agency committees.
QUALIFICATIONS (Education and Experience):
- Diploma in a related human/social service field and/or equivalent education and skills working in asocial service field.
- Must have a minimum of two years’ experience working with vulnerable seniors.
ADDITIONAL REQUIREMENTS:
- Clean Criminal Record
- Current First Aid Training and CPR
- Must have a valid unrestricted Class 5 B.C. Driver’s License
- A clean driver’s record check (annually)
- Must have an available, reliable vehicle and vehicle liability insurance in accordance with
Community Services policies and procedures
Shift/Schedule/Hours per week: Full-time, 35 hours per week
Expected Start Date: ASAP
Wage/Salary: $29.17 /hour
Grid Level: 12
