We’re looking for a compassionate, collaborative professional to join our Social Prescribing team—someone who sees the strengths in every senior and is committed to advancing health and well-being through inclusive, person-centered supports.
At Community Services, we believe in a holistic approach to health. Our Social Prescribing Program works alongside clinical care to support seniors in accessing the full spectrum of community resources that enhance physical, mental, and emotional well-being. From nutrition and exercise to social connection and caregiver support, we walk alongside seniors as they explore what matters most to them.
**ABOUT THE ROLE**:
You’ll provide culturally responsive, strength-based support to seniors through meaningful relationships, health and wellness planning, and coordinated referrals. This includes:
- Partnering with seniors to complete intake assessments, co-create wellness plans, and conduct regular follow-ups, including frailty assessments every 3 months.
- Building strong connections with seniors through home visits and ongoing support grounded in respect, autonomy, and dignity.
- Collaborating with health care providers and other community connectors to ensure seamless and responsive care.
- Identifying and coordinating access to a wide range of community services—both formal and informal—that reflect each senior’s unique strengths, needs, and cultural background.
- Creating presentations and sharing program impacts with funders, health professionals, and other stakeholders to raise awareness of the Social Prescribing model.
- Participating in Communities of Practice and meetings with the United Way and other partners to share learning and amplify collective impact.
- Contributing to a supportive team environment by working closely with colleagues across our Seniors Department to plan events, recognize volunteers, and continually improve our programs.
**WHAT YOU BRING TO THE ROLE**
- Deep understanding of the diverse experiences and barriers seniors may face, particularly those who are vulnerable or at risk of social isolation or frailty.
- Minimum two years’ experience supporting seniors, with demonstrated ability to create individualized wellness plans and connect people to appropriate resources.
- Comfort with in-person outreach and home visits, grounded in a strengths-based and trauma-informed approach.
- Experience using platforms such as BC Healthy Aging CORE and iUnite for reporting and coordination.
- Proven skills in program reporting, grant writing, and stakeholder presentations, including funders and healthcare professionals.
- Strong knowledge of local, provincial, and federal seniors’ services, with a commitment to lifelong learning and cultural humility.
- Ability to collaborate across sectors and with diverse stakeholders to create inclusive, responsive systems of support.
**WHAT WE’RE LOOKING FOR**
- Education and Experience - You bring a diploma in a related human or social services field, or you’ve gained equivalent knowledge through a meaningful mix of education, training, and lived or professional experience.
- Experience Supporting Seniors - You have at least two years of hands-on experience working directly with older adults, and you understand the importance of building trust and connection.
- Inclusive, Compassionate Approach - Your work is grounded in trauma-informed, culturally safe, and strengths-based practices that honor each person’s unique story.
- Equity-Driven Mindset - You’re committed to fostering accessibility, inclusion, and equity in every aspect of your work—and you actively seek to break down barriers for those who need support most.
**ADDITIONAL REQUIREMENTS**
- Clear Criminal Record Check is required
- Current First Aid & CPR Certification
- Valid Class 5 BC Driver’s License and access to a reliable vehicle with appropriate insurance
- Annual driver’s abstract
- Position requires union membership (CUPE)
**Shift/Schedule/Hours per week**:30 hours per week; Monday to Thursday, 8:30 am to 5:00 pm
**Expected Start Date**:ASAP
**Wage/Salary**: $29.17 /hour
