**Overview**
The Housing Case Manager works in a team providing high-quality case management services to assist tenants in maintaining stable long-term housing. The Housing Case Manager will have a thorough understanding of harm reduction and housing first philosophies and will be able to demonstrate this verbally and in practice.

Key Responsibilities:

- Conduct intake assessments to determine housing needs, eligibility, and barriers to housing stability.
- Meet individuals where they are at, providing non-judgmental and empathetic support
- Develop individualized housing and service plans in collaboration with clients.
- Provide ongoing case management, including home visits, crisis intervention, and referrals to supportive services (e.g., mental health, substance use treatment, employment assistance).
- Maintain accurate and timely documentation in case files and databases
- Advocate for clients’ rights and access to services within the housing system.
- Collaborate with internal teams and external partners to coordinate care and resources.
- Monitor client progress and adjust service plans as needed to support housing retention.
- Participate in team meetings, trainings, and community outreach events.

The Housing Case Manager will have specific accountabilities which will include regular assessments, developing action plans, crisis counseling, home visits, unit maintenance support, and goal setting.

The Housing Case Manager will have a reliable vehicle, with appropriate insurance, as they will be providing transportation to certain appointments for tenants, and as necessary.

The Housing Case Manager will maintain case notes and other documentation in tenant case files, contribute to statistics for monthly reports; and other duties that might be required as part of the John Howard SENB team.

The goal of this position is to assist individuals within the housing program in maintaining their tenancy by offering comprehensive wraparound services, including mental health support, addiction recovery resources, and life skills training while fostering the tenants' independence.

Qualifications:

- University degree or college diploma in Social Science, Family or Human Services. Combination of experience and training may be considered.
- Experience with and/or lived-experience with marginalized populations with the ability to empathize with a wide variety of challenging experiences in a caring, non-judgmental manner.
- Demonstrate an understanding of and respect for the roots of the issues that may be presented by tenants
- Knowledgeable in motivational interviewing skills and trauma informed care models. Knowledge of systems and program referrals within the community that are applicable to the tenant and their case plan
- Ability to work independently and manage a caseload effectively.
- Proficiency in Microsoft Office
- Bilingual in French and English is an asset
- Excellent interpersonal, communication and listening skills
- Basic CPR/First Aid/ ASIST are assets

In addition, due to the changing nature of the work, ongoing professional development will be required.

A car and valid driver's license is required.

This is a full-time position (minimum of 37.5 hours/week) with some evenings and weekends as required.
