SickKids serves a diverse population of patients and families dealing with acute, complex, and specialized physical and mental health disorders. A substantial increase in volumes have been and are continuing to be observed across inpatient, outpatient, and emergency mental health services since 2015. In response to this growing need, SickKids is embarking on an ambitious and exciting plan to develop and expand our mental health services, where there will be significant impact on the Department of Psychiatry.

Reporting to the Senior Clinical Manager of Outpatient Psychiatry, working collaboratively with the interdisciplinary Mental Health Inpatient Senior manager and the Mental Health Access Program (MHAP), this case navigator role will **support patient flow** for inpatients followed by the Psychiatry service across the organization, **review and organize bed transfer requests** to the inpatient mental health unit (internal and external), **assist in coordinating patient transfers** to other facilities/hospitals, **provide guidance around resources** to best support patient care and flow and ensure patient care adheres to the standards of Mental Health Act of Ontario.

**Here's What You'll Get To Do**:

- Provide bed flow support for both the Eating Disorders and Psychiatry patients who are cared for off-service or in a cohort unit
- Lead daily planning to support off-service patients of the Mental Health and Eating Disorder program
- Attend bed flow meetings when required
- Support CYC assignments and support the point of care teams
- Enable and manage transfer processes/packages within the hospital and to community partners for both populations
- Lead communications and respond to unit inquiries and needs
- Review Mental Health Act Forms to ensure quality control
- Liaise with the following teams: Mental Health Inpatient Unit, CHS Administrator, Bed Flow coordinator, Consultation Liaison Service and the Mental Health Access Program with the goals of co-ordination of care, bed flow and communications with multiple stakeholders essential to patient management and bed flow
- Document effectively in the electronic health record or record the progress of the patients

**Here's What You'll Need**:

- Previous coordination experience in hospital clinical processes and inpatient flow and care at SickKids
- Experience with mental health and eating disorders populations
- Mental health background such as a Child and Youth Counselling Diploma from a community college with a Baccalaureate degree in a related field (preferred)
- Professional membership with the Ontario Association of Child and Youth Counsellors (OACYC)
- Excellent communication and therapeutic engagement skills particularly with youth and families
- Understanding of culturally competent care and trauma informed care
- Knowledge of major domains of major mental illness such as mood and anxiety disorders as well as eating disorders
