**CASE MANAGER - YOUNG ADULTS (18-24) - Partner Assault Response Program**

**About Changing Ways**: Changing Ways challenges the roots of gender-based violence through prevention, education, collective action, and building hope. We provide interventions and support that disrupt violence and destructive thoughts and behaviours that cause harm in intimate relationships, with a strong focus on supporting survivors and victims as an embedded part of our programming.

**Position Overview**:
In the partnership with the London Justice Centre and other community-based partner organizations, the Case Manager for Young Adults (18-24) will provide intensive, holistic, wrap around supports that address intersecting risk factors that lead to intimate partner violence. The Case Manager will be experienced in developing trusting and respectful relationships with young adult populations, utilizing a trauma informed approach, assessing and safety planning for dynamic risk factors, and supporting connection & engagement with community resources that promote safety, harm reduction, and personal goal development.

**Key Responsibilities**:

- **Intensive Case Management for Young Adults (18-24)**with Intimate Partner Violence criminal charges proceeding through the London Justice Centre court.
- **Supporting Partner Assault Response Group Facilitation for Young Adults (18-24)** delivered over a 12-week period, utilizing a psychoeducational approach, and prescribed session topics adapted to a young adult population.

**Core Competencies**:

- deep understanding of intimate partner violence (IPV) from a gendered perspective, including coercive control dynamics, and the complexities of victimization and perpetration;
- skill in creating safe, trauma informed, supportive environments for young adults utilizing a holistic approach to build engagement;
- specialized knowledge of the challenges with intimate partner violence offending behaviours and attitudes, and the safety risks posed to victims and survivors of IPV;
- case management experience, including facilitating multi-sectoral needs assessments and making needs-based, strengths-based, and trauma-informed connections to supports, services, and programming tailored to justice-involved young adults;
- demonstrated ability in dynamic risk assessment, safety planning, crisis support, counselling, and system navigation;
- experience working within a multidisciplinary team, in collaboration with justice and community partners for holistic client care;
- strong knowledge of the criminal justice system and understanding of community and criminal justice services in their area - including familiarity with the London Justice Centre court;
- awareness and sensitivity to intersecting oppressions impacting diverse client demographics, and commitment to addressing intersecting oppressions and allyship to the 2SLGBTQIA+ community;
- lived or learned experience working with young adults and the criminal justice system;
- capability to provide compassionate support while maintaining professional boundaries;
- responsive adaptability to dynamic, fast-paced, work environments with a focus on the safety and wellbeing of justice-involved individuals;
- excellent communication and relationship building skills with both internal and external stakeholders;
- excellent self-care practices, professionalism, and ethical accountability; and
- strong leadership, critical thinking, and communication skills.

**Responsibilities**:

- increase awareness of the right to counsel for justice-involved young adults;
- facilitate an intake and assessment process to program participants, which will identify their needs and strengths, and forms the basis of developing connections to multi-sectoral, community-based, supports, services, and programming;
- connect program participants to community-based services and supports based on an intake and assessment process, with input from the Justice Centre Crown;
- support the program participant’s engagement with community-based services, supports, and programming through the steps including, but not limited to, making referrals based on the intake and assessment, support court appearances, booking appointments, providing appointment reminders, and following up with the applicable service providers;
- provide, through regular meetings at the request of the Justice Centre Crown, progress updates on the participant’s engagement with community-based services and supports for the efficient and timely disposition of court matters;
- proactive communication to management, and the Justice Centre Crown, on emerging issues, workload, scheduling, absences, challenges and barriers to provision of service as well as requests from the court;
- provide aftercare supports to program participants after the disposition of their criminal matter; provide administrative support, including maintaining all records and files, relating to program participation, their engagement, referrals made
