Client Service Coordinator

**CLIENT SERVICE COORDINATOR**
**Full Time - 3 Month Contract**
**37.5 hours per week**
The Client Services Coordinator is responsible for the intake and assessment of new clients, introducing new clients to the programs of ICAN, and serving as an informational resource for clients. The Client Services Coordinator is also responsible for providing support to staff by communicating client needs.
- Process referrals from multiple sources - Act as a resource for staff and clients
- Conduct in home visits with clients - Assist clients to access the community,
- Develop service plans, service maintain independence and promote
agreements and schedules of service self-advocacy
- Conduct reassessments - Provide backup to RSWs when
- Maintain client files necessary
- Completion of ADTs in timely manner - Other duties, as required
**Qualifications/Specifications**:
- Ability to meet the scheduling requirements of the position
- Post-secondary education in health, community, or social services
- Experience working with people with physical disabilities or in a long-term care setting preferred
- Current First Aid and CPR Certificate required
- Highly developed communications skills; bilingualism is an asset
- Highly developed computer skills
- Excellent organizational skills, time management skills, and knowledge of relevant legislation
- Ability to work both independently and collaboratively
Salary Range: 20.29-23.41 per hour
