Cornerstone to Recovery is seeking a Program Facilitator responsible for delivering comprehensive client-centered support and case management services while managing various operational and administrative functions within the organization. This role combines direct client service delivery with program coordination, community outreach, facility oversight, and wellness support. **Job Details**:

- 40 hours/week, Monday to Friday, 8 am to 4 pm
- Full-time, permanent role
- Salary, commensurate with experience
- Benefits after successful 3-month probationary period

**Experience Required**:

- Diploma, Bachelors or Certificate(s) in addictions and mental health counselling, social service work or another related field and/or actively working towards the completion of education.
- Experience and confidence with windows computers, outlook, word, PowerPoint and excel is required.
- Experience with employment and life skills coaching is an asset
- First Aid and other relevant certifications is an asset.
- Lived experience with addiction recovery is an asset.
- Experience working or volunteering in the field of addictions is an asset.
- Knowledge and familiarity with the 12 step model and abstinence based recovery model is an asset.

**Key Responsibilities**:
**Program Delivery & Case Management**
- Facilitate the Day Recovery Program as needed, ensuring consistent, high-quality service delivery.
- Engage with current and past clients to provide client-centered support, referrals, and treatment planning.
- Collaborate with community partners to coordinate referrals, advocate for client needs, and address barriers to services.
- Complete client intakes and assessments.
- Provide case management, including the development and implementation of individualized treatment and support plans.
- Monitor client progress and adjust service plans as required.

**Walk-In Management & Client Services**
- Greet walk-in clients, assess immediate needs, and discuss service preferences.
- Collaborate with clients to identify available options and create action plans aligned with client goals and safety.
- Provide appropriate referrals to internal and external services.
- Conduct follow-up check-ins on client progress.
- Deliver client tours, orientations, and registration packages for new members.

**Community Outreach & Partnerships**
- Build and maintain relationships with community stakeholders to enhance service coordination.
- Represent the organization at community events and meetings as required.

**Administrative Functions**
- Answer and direct incoming phone calls, responding to inquiries or taking messages as needed.
- Maintain accurate and organized filing systems for client records, program documentation, and administrative materials.
- Compose and send professional correspondence to community agencies, partners, and stakeholders.
- Regularly post and update promotional flyers within facilities.

**Facility Management**
- Ensure the cleanliness and maintenance of facilities and equipment.
- Oversee the coffee station, ensuring fresh coffee is available.
- Keep flyers, brochures, and informational materials current, relevant, and well-displayed.

**Residential Support Services**
- Track alumni and conduct annual check-ins with program graduates.
- Collaborate with the counseling team to provide wraparound support to residential clients.
- Coordinate services and resources to support successful program completion and transitions.

**Fundraising & Special Events**
- Participate in the planning and execution of fundraising events.
- Volunteer at organizational fundraising activities, including bingo events.
- Support promotional and marketing efforts for special events.

**Wellness & Volunteer Support**
- Communicate with the Senior Director regarding volunteer inquiries, needs, and contact information.
- Contribute to maintaining a therapeutic, supportive, and welcoming environment for all clients.

Pay: $38,000.00-$40,000.00 per year
