Family Resource Network Navigator

The Family Resource Navigator provides direct service delivery to eligible participants and is responsible for the effective planning, implementation and evaluation of program services. The Family Resource Navigator will demonstrate excellent interpersonal and communication skills with diverse populations who face unique challenges. The Family Resource Navigator will provide services through in-reach and outreach approaches to best meet the needs of families and to reduce barriers to support. The Family Resource Navigator carries out their duties in a professional manner and in accordance with all YMCA policies and procedures, standards of conduct and practice, health and safety standards, child protection standards and contractual, legal and regulatory requirements.

**Hours of Work**:Full time with flexible evenings and weekends
**Accountability/Responsibilities**:
1. Program and Service Delivery
- Comply with funder mandates for participation and promotion requirements.
- Collect information to confirm eligibility and complete participant intake and/or assessment to inform ongoing participant supports
- Triage families, follow case management practices as per Standards of Practice guidelines and funder mandates, to support warm transfers of participants to internal and external programs
- Plan, implement and evaluate services/workshops as per Standards of Practice guidelines and funder mandates
- Maintain knowledge of relevant services and community resources related to the Social Determinants of Health
- Support participants and their natural supports within YMCA guidelines and adhere to the contracted performance outcomes.
- Engage with community agencies, and other organizations to maximize services and resources to participants
2. Program Administration
- Comply with all Risk Management and Occupational Health and Safety requirements including Work Alone Policy and Procedures and the Children, Youth and Vulnerable Adults Protection Policies and Procedures.
- Inputs data accurately and timely into program designated database systems
- Work within defined program budgets
- Maintain participant files in accordance with funder and YMCA guidelines
- Collect data, including outcomes, to inform and contribute to the development of funder and YMCA reports.
**Qualifications**:
- ** Competencies**:Communication, Systems Thinking, Relationship Building, Decision Making plus competencies consistent with CHI Standards of Practice
- ** Education/Certifications/Credentials/Professional Registration**
- University Degree in Human Services field or equivalent.
- Annual registration in a professional organization as required by law.
- Standard First Aid and CPR-C, Police Intervention Check with Vulnerable Sector Search and where required Child Intervention Check
- WHMIS Certification
- Non-Violent Crisis Intervention and Suicide Intervention Certification preferred.
- Valid driver’s license and access to a vehicle. Where required a clear driver’s abstract and proof of liability insurance is needed.
- ** Experience**
- 2 years of experience working directly with families with multiple risk factors
- Experience working with diverse populations.
- Experience within a team environment and working remotely.
- ** Knowledge/Skills**
- Working knowledge of issues related to the social determents of health, individual assessment, case management, group work, group facilitation, program promotion, program planning and outcome evaluation
- Working knowledge in supportive counselling skills, culturally sensitive practice and trauma-informed, strength-based approach, and Well-being and Resiliency Framework
- Working knowledge of related community, housing, education, employment, justice, family and human services resources.
- Excellent organizational and time management skills.
- Excellent communication, interpersonal, and team skills.
- Competencies consistent with CHI Standards of Practice.
- Working knowledge computer skills (Windows, Microsoft Office and Outlook)
