Seniors System Navigator: Social Prescribing

**Responsibilities**:
1. Complete psychosocial assessments (as required), identify issues and assist with connection to applicable community resources
2. Utilize a case management model to provide comprehensive service planning, coordination & implementation of early intervention strategies
3. Provide client centered, goal orientated psychosocial support (emotional and practical) focused on improving or maintaining emotional well-being, relationships and quality of life
4. Comfort providing 1-1 companion-like support to older adults to build rapport and encourage/facilitate improved socialization
5. Facilitate support groups as needs are identified
6. Provide referrals to LSCO programs as well as other community agencies
7. Actively liaise with senior serving, community & health organizations to address issues
8. Provide outreach assistance with identified activities of daily living
9. Responsible for all data entry, case notes and reports pertaining to position
10. Comfort transporting older adults as needed, when safety has been determined
**Qualifications**:
1. Bachelor of Social Work (BSW) with RSW preferred
2. Three years of experience working with the senior population and/or relevant case management experience preferred
4. Able to work independently but collaborate within interagency multidisciplinary teams
5. A broad knowledge of personal and social issues affecting older adults
6. Must have adequate experience and skill with personal computers and office productivity software
7. Valid driver’s license. $2 million liability Insurance required. Vehicle required
**Job Types**: Full-time, Fixed term contract
Pay: $25.00-$27.00 per hour
Expected hours: 37.5 per week
