**Reports to**: Seniors System Navigator Team Lead
**Position Summary**:
Intake is the first point of contact for those seeking information on support services through the Seniors Community Services Partnership and the LSCO.
**Detailed List of Responsibilities**:
To perform this job successfully the individual must be able to achieve the responsibilities listed below with excellence, focusing on work quality, timeliness, and precision.
All LSCO employees are expected to perform responsibilities to meet organizational and funding requirements as directed by mandate and policy.
**Client & Program Support**:
- Complete phone and in person intakes at the LSCO and any other community agencies as assigned;
- Provide information, referrals, and simple interventions to older adults deemed as low acuity;
- Identify older adults in need of higher acuity support, complete an intake, and then refer them to SSN Team Lead;
- Support the coordination and delivery of efforts targeting SCSP program events as needs present themselves (ie. education/support groups) including promoting and hosting a variety of socialization opportunities within LSCO to establish an improved sense of community.
- Remain knowledgeable of legislation and regulations relevant to operations (Personal Directive Act, Social Housing Act, Protection for Persons in Care Act etc.).
**Community Partnerships**:
- Establish partnerships with the community (groups, associations and institutions), LSCO members, and volunteers to engage them in supporting seniors focused initiatives and strengthen the SCSP.
- Effectively communicate to community stakeholders about SCSP programming and support services opportunities;
- Provide education opportunities to community members and agencies about the SCSP partnership and topics that impact older adults;
- Compile and disseminate the monthly support services calendar to community stakeholders.
**Administration**:
- Support quarterly collection of data required by funders;
- Responsible for all data entry, case notes, and reports pertaining to position;
- Other duties as assigned.
**Education & Qualifications**:
- Bachelor of Social Work (BSW) and registered with Alberta College of Social Workers, or related degree or diploma or a relevant equivalency of education and experience considered;
- Experience working with the senior population and/or relevant case management experience;
- Ability to prioritize presenting needs of older adults;
- Able to work independently but collaborate within interagency multidisciplinary teams;
- A broad knowledge of personal and social issues affecting seniors;
- Must have adequate experience and skill with personal computers and office productivity software;
- Valid driver’s license, vehicle preferred;
- Current Criminal Record Check (within 90 days of employment).
**Job Types**: Full-time, Fixed term contract
Contract length: 18 months
Pay: $27.00-$29.00 per hour
