Your Opportunity:
The Social Worker II contributes to the reduction of disparities experienced by vulnerable populations living with chronic disease and the development of Alberta Health Services as a diversity competent health care organization, to exert a positive impact on the health of clients and families. The Social Worker facilitates connections between staff, programs and services and vulnerable clients/families/communities for the purposes of increasing communication, understanding, access and coordination. The Social Worker collaborates with Alberta Healthy Living staff and multidisciplinary teams to plan, implement and evaluate initiatives to ensure they are accessible to clients/families and communities. The Social Worker contributes to engaging and strengthening collaborative community partnerships to collectively identify ways to increase the capacity of: 1) existing services to better meet the needs of vulnerable populations; and 2) vulnerable clients/families/communities in accessing health services and information to meet their needs. Through both individual and group programming, the Social Worker will provide a range of navigating, referral, education and support services to chronic disease clients/families by providing mental health care and support that is culturally, linguistically and otherwise appropriate to chronic disease populations through a variety of strategies.

**Required Qualifications**:
Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
**Additional Required Qualifications**:
Minimum of 2 years' experience working in a community health setting.
**Preferred Qualifications**:
Two years related experience in a health care setting and/or community-based health, human or social services related setting related to providing mental health supports. Experience communicating and liaising between members of vulnerable populations and health care providers/professionals and community organizations. Experience in planning, developing, delivering and evaluating group education sessions.
