**ADDICTION CASE MANAGER**

**FOR PREGANCY/PARENTING OUTREACH PROGRAM**

**Full Time - 40 hours/week**

Monarch is seeking one Permanent Full-Time worker (40 hrs./week) to perform the job duties of an outreach worker, providing addiction services to pregnant women and or parenting individuals with young children.

**Responsibilities**
- Provide case management services to individuals in the PPOP program, ensuring effective communication and support.
- Conduct assessments and develop personalized treatment plans for clients.
- Maintain accurate documentation and case notes.
- Collaborate with multidisciplinary teams to facilitate access to resources and services.
- Effectively communicate with clients and their families.
- Monitor client progress and adjust plans as necessary, demonstrating strong time management and analysis skills.

**Qualifications**:

- Bachelor degree from an accredited university in human services, with appropriate experience in counselling
- Experience and knowledge of addictions, trauma-informed
- Excellent written and oral communication skills
- Must have own vehicle, valid driver’s license
- Proficiency in computer Word program
- Ability to speak in both official languages an asset

**Job Types**: Full-time, Permanent

Pay: $26.52-$27.77 per hour

Expected hours: 40 per week
