**ADDICTION SUPPORTIVE HOUSING - CASE MANAGER**

**Full Time - 40 hours/week**

Monarch is seeking two Full-Time/Contract worker's (40 hrs./week) to work as a case manager with individuals struggling with substance use and to maintain tenancy.

**Responsibilities**
- Assist individuals in finding and maintaining permanent housing.
- Provide case management services to individuals in the ASH program, ensuring effective communication and support.
- Conduct assessments and develop personalized treatment plans for clients.
- Maintain accurate documentation and case notes.
- Collaborate with multidisciplinary teams to facilitate access to resources and services.
- Effectively communicate with clients and their families.
- Monitor client progress and adjust plans as necessary, demonstrating strong time management and analysis skills.

**Qualifications**:

- Bachelor degree from an accredited university in human services, with appropriate experience in counselling
- Experience and knowledge of addictions, trauma-informed
- Excellent written and oral communication skills
- Must have own vehicle, valid driver’s license
- Proficiency in Micro Soft
- Ability to speak in both official languages an asset

**Job Types**: Full-time, Contract
Contract length: 36 months

Pay: $27.32-$28.61 per hour

Expected hours: 40 per week
