**POSITION OBJECTIVE**

The Dual Diagnosis Justice Case Management program requires one (1) full-time contract Dual Diagnosis Justice Case Manager to work in the Brant, Hamilton, Haldimand, Norfolk, and Niagara communities.

Working to promote Bethesda’s values, vision and mission statement and quality client care, The Dual Diagnosis Justice Case Manager will provide case management working with justice, mental health and development services to develop case plans for high needs individuals involved with justice, mental health and developmental service sectors.

**Key Job Responsibilities**
- Coordinate the discharge planning process by partnering and planning with mental health court support workers, local correctional workers and custody facilities.
- Coordinate a continuum of services based on client choice and need; act as the central contact person to be referred to mental health services and other supports; work with individuals receiving service to maintain community involvement with adequate supports.
- Act as a link between the Southern Network of Specialized Care in the region and the developmental services case management supports at the community level to facilitate longer term support for individuals.
- Participate in regular meetings with mental health court workers to share information and identify issues and strategies to improve their services.
- Act as a resource to justice and mental health systems regarding service delivery and gaps in the developmental service and provide input into resolving process issues.
- Plan for ongoing Ministry of Community and Social Services case management services with local communities for individuals.
- Identifies, informs and facilitates the development of a plan that will address the mental health needs, developmental and community support needs at key intervention points in the judicial process.
- To provide transitional support to the target population.
- Comply with Bethesda policies and procedures relating to health and safety.
- Responsible for reporting all health and safety concerns immediately following the Internal Responsibility System.
- All other duties as assigned.

**QUALIFICATIONS**
- Degree in Social Services, Social Work, Criminology, Justice or other related field.
- A minimum of five years previous experience working within the Developmental Services Sector, Justice and/or Mental Health field.
- In-depth knowledge of the Developmental Services Sector, Justice and/or Mental Health field.
- Proficient with Microsoft Office.
- Valid Driver’s License.
- First Aid Training and CPR Certification.
- Safety Care certified.
- Professional Communication Skills: professionally represents the company through strong communication skills, both oral and written.
- Teamwork: excels at working with others and communicating with a team to provide the most effective support to clients.
- Problem-Solving: able to navigate challenges and barriers, always looks for potential pitfalls and risks to help mitigate problems ahead of time, and knows when to ask for help.
- Proactive: anticipates and solves problems and issues before they are brought to your attention
- Fluent in French is an asset
