- 3 weeks’ vacation (after mandatory completion of waiting period)
- Provide trauma informed, intensive case management services to women, families, and youth who are experiencing chronic homelessness
- Work with service users in the community to secure permanent housing and will provide ongoing case management supports for up to 24 months
- Will partner with various agencies to leverage and coordinate the assets and services available in Hamilton to ensure that service users have their individualized housing and support needs met
- Work takes place in the office, overflow hotels, emergency shelters, and in the community
- Accompany service users to community appointments, apartment viewings, and will attend service users’ homes for monthly home visits
- Must be willing and able to assist service users in a hands-on capacity
- Time management skills are necessary in a high energy, changing environment
- Ability to work independently

**Qualifications**
- Minimum 1 year related experience or equivalent combination of education, training and experience in Social Services or related field
- Fluency in a 2nd language is an asset
- Proficiency in Microsoft Office programs and strong written and oral communication skills required
- CPR certification or willingness to be certified is required
- Valid G license required
- An in depth understanding of trauma, addictions, mental health, and homelessness is required
- Creative problem solving and advocacy skills required
- **Hours of Work**:Shifts are day shifts (9 am to 5 pm), one evening shift per week (11am to 7 pm) and one Saturday approximately every six weeks (9am to 5pm)
