Regional Support Associates, in association with Woodstock Hospital, offers inter-disciplinary clinical and specialized services informed by our expertise within Behavioural Consultation, Case Management, Psychology, Psychiatry, Primary Health Care, Speech and Language Pathology, Occupational Therapy, Social Work and trauma informed care for people with intellectual disabilities across the Southwest Region of Ontario.
- Coordinating supports and services within and across sectors including Ontario’s healthcare system for people with complex multisystem needs.
- Focusing on the person, offering direct services to assist with a person’s goals as a way for capacity building and bridging within and between service systems.
- Acting as a resource to service agencies, the DSO and local planning tables.
- Knowledge in the field of social and health sciences as would normally be acquired through the completion of a degree or diploma from an accredited School of Nursing.
- Registered or eligible for registration with the College of Nurses of Ontario.
- Preferred minimum of 3-5 years’ experience in the Children’s or Adult Developmental Services Sector, Mental Health of Health services sector.
- Excellent interpersonal, written and verbal communication skills are mandatory, as is demonstrated ability to deal congenially with a variety of people effectively and efficiently with mínimal supervision.
- Excellent problem-solving skills.
- This position requires an individual with demonstrated case management and leadership abilities in the community.
- Ability to independently plan & prepare presentations and proposals.
- Strong understanding of and commitment to a service coordination model.
- Existing knowledge of and ability to access community resources.
- Satisfactory vulnerable person’s sector criminal background check.
- Comfort working in a fast-paced, multi-tasking environment with expectations of program deadlines and service targets.
- The ability to succeed both independently and collaboratively with internal and external professionals as required in multi-sectoral settings.
- A valid Driver’s License, with a minimum G2 standing, and access to a reliable automobile for extensive travel throughout Southwestern Ontario.
- Bilingualism (English/French) is an asset.
