**Position Summary**:Behavioural Supports Ontario (BSO) provides enhanced, integrated, and cross-sector services that meet the needs of older adults with responsive behaviours associated with dementia, mental health, addictions, and other neurological conditions, in the community, long-term care (LTC) or hospitals and Community. **The Behavioural Supports Ontario (BSO) Personal Support Worker (PSW),** as a member of the BSO LTC Lead Team, provides support for residents with responsive behaviours for the LTC Homes in the Chatham-Kent area. The Erie St Clair BSO framework for the LTC homes asks that there be a collaborative approach to supporting responsive behaviours, thereby increasing their quality of life. Integrated services include System Navigators, LTC Internal Champions, LTC Lead Teams, Geriatric Mental Health Outreach Teams (GMHOT), Hospital Navigators, Clinical Care Coordinators RN’s, Primary Care Providers, and Psychiatrists.
**Key Responsibilities**:
- Provide front-line support to residents with various dementias, delirium, mental health, and other conditions resulting in behavioural changes.
- Coach and mentor team members and colleagues at LTCHs in evidence-based care.
- Utilize a problem-solving approach when behaviours present, within the scope of PSW practice.
- Collaborate and communicate with BSO Partners (i.e. GMHOT, NLOT, Family Service Kent, etc.) and other allied health care professionals to provide wraparound care
- Demonstrate knowledge of Alzheimer’s disease and related dementias, mental health, and other conditions. Upgrade skills as required.
- Develop and maintain an effective working relationship with each LTC home.
- Participate in educational in-services offered to all LTC Homes by the BSO LTC Lead Team.
- Document all findings and interventions in a clear and concise manner in the resident’s clinical record (as allowed in each LTC Home).
- Advocate for and protect the individual rights of residents/clients.
- Participate in Quality Improvement activities in areas LTC Homes as needed or requested.
- Demonstrate effectiveness as a BSO LTC Lead team member through collaboration, respect, and effective communication skills in interacting with residents, family members and other members of inter-professional care teams.
- Demonstrated ability to handle demanding workload effectively and efficiently.
- Ability to conform to shifting priorities, demands and timelines through problem solving capabilities.
- Ability to adapt positively and productively to changes in the work environment.
- Excellent coaching/mentoring skills
- Proficient computer skills. (Microsoft products, Zoom, etc.)
- Fulsome understanding of health equity principals inclusive of cultural humility and sensitivity
**Qualifications**:
- Certified as a Personal Support Worker (PSW)
- Current CPR certification
- Grade 12 Diploma or equivalent
- Training in Gentle Persuasive Approaches (GPA) and U-First an asset
- Minimum one (1) year experience with clients in long-term care or residents in a long-term care home setting
- Experience working with persons affected by Alzheimer’s disease or related dementias an asset.
- Demonstrated resident-centered focus and commitment to the principles of the BSO Action Framework of Care
- Commitment to ongoing training, including U-FIRST/ GPA and others and quality improvement initiatives (QI)
- Demonstrated effectiveness working with mínimal supervision with proven reliability and trustworthiness.
- Proven good attendance record.
- Excellent verbal and written communication skills in English
- Criminal Records Check and Vulnerable Sector Screening
- Knowledge of PCC/RAI MDS an asset
- Knowledge and ongoing practice of person-centered care.
- Valid driver’s license.

**Hours of Work**:
- Monday to Friday 8:30 - 4:30pm
- Flexibility in work hours (potential for on-call responsibilities and/or weekend work)
**Work Conditions**:
- Physical ability to fully participate in meetings / case conferences in multiple health sectors (i.e., Hospital, LTC, Retirement Home)
- Extensive travel throughout C-K area.
**Job Types**: Full-time, Permanent
**Salary**: $16.00-$22.00 per hour
