**Role Overview**
- As the Referral and Placement Coordinator for Cota's Transitional Age Youth (TAY) Program, your role is integral to the effective functioning of the program, ensuring a seamless transition for service users referred by child welfare agencies. You will not only oversee the critical administrative aspects of referral processing, budget negotiation, and placement coordination but also play a key role in fostering a supportive and culturally sensitive environment for service users. Your dedication to adhering to agency policies, promoting person-centered services, and maintaining strong communication channels will contribute significantly to the success of the program.

**About the Team**
- The Referral and Placement Coordinator work alongside the TAY Team; including Managers, Case Managers, and Cota Finance Team. This program supports adults with developmental disabilities who were formerly in the care of one of the Toronto-based Children’s Aid Societies. The program ensures that clients receive quality support while they reside in privately operated fee-for-service residential care settings The TAY Team is currently comprised of 13 Case Managers, 3 QAM Advisors, a Quality and Risk Advisor, and 2 Managers.

**Primary Responsibilities**:

- Oversee the administrative processing of incoming referrals to Cota's TAY Program from child welfare agencies.
- Assign new clients to case management team.
- Collaborate in negotiating budget proposals for new service users, utilizing a standardized framework with defined budget ranges.
- Engage in budget negotiations with external paid resources for current service users.
- Ensure all clinical information is provided and assess service users’ clinical information to determine the most suitable budget considerations.
- Collaborate with the administrative assistant to facilitate the submission of all new contractual agreements through the DocuSign platform.
- Provide assistance in clarifying roles, handling financial inquiries, ensuring adherence to agency policies, and facilitating service agreement discussions.
- Collaborate with the administrative assistant to facilitate the completion of onboarding processes for new agencies.
- Generate and submit regular forecasting reports to the Ministry of Children, Community, and Social Services.
- Collaborate closely with the finance team to jointly project and estimate upcoming expenses, customizing the approach to meet the specific needs of clients within each cohort.
- Provide assistance to case managers in handling emergency placement admissions.
- Regularly liaise with external paid resources to assess placement availability.
- Emphasize attention to cultural, racial, and identity considerations throughout the placement planning process to meet the needs of the service user.
- Sustain consistent communication and liaison efforts with child welfare agencies and Cota’s management team.

**Other Responsibilities**:

- Adheres to all Cota’s policies and procedures and follows established applicable program guidelines, procedures and practices.
- Adheres to all applicable legislation, in particular, the Ontario Human Rights Code.
- Complies with all Health and Safety policies and procedures to ensure the protection of workers and clients and that health and safety concerns or incidents are responded to promptly.
- Works cooperatively, respectfully and constructively with clients, team members, management, other service providers, and external stakeholders.
- Ensures that confidentiality of clients, employees and Cota information is maintained at all times.
- Undertakes other related and/or delegated duties, as assigned.
- A post-secondary degree/diploma in a related field.
- Minimum of 2-3 years of work experience specifically and primarily with adults with developmental disabilities.
- Expertise in trauma-informed clinical knowledge.
- Familiarity with financial reporting.
- Previous training or work experience within the mental health sector and/or the justice system would be considered assets.
- Experience working with external organizations and building positive working relationship
- Strong clinical skills to accurately analyze and interpret clinical data.
- Knowledge of and commitment to person-centered services.
