**Position Summary**

Reporting to the Manager, Comprehensive Intake and Care, the Housing Client Intervention Worker will support seniors and adults with disabilities and illness secure and maintain appropriate housing.

The Housing Client Intervention Worker is required to perform his/her duties in a manner consistent with Dixon Hall’s Mission, Vision and Values, ensure safety, and demonstrate customer service excellence.

**Duties and Responsibilities**
HOUSING-FOCUSED CASE MANAGEMENT SUPPORT:

- Assist clients in the development of an individualized plan of action with short and long-term goals and identified strategies to achieve housing stabilization.
- Identification of physical, mental, socioeconomic and environmental factors contributing to precarious housing.
- Make referrals to appropriate services based on clients’ needs.
- Advocate on behalf of clients in hospital, residential and legal settings.
- Liaise with property management, legal professionals, health care professionals and other community partners.
- Inform clients of their rights and responsibilities.
- Work as a member of an interdisciplinary outreach team providing comprehensive support with the goal of securing and maintaining housing.
- Liaise with other internal or external services to identify strategies and enhance activities towards supporting the goal of maintaining housing.
- Arrange and maintain regular and intermittent times for home visits with clients.
- Accompany clients to appointments (i.e. medical, legal) where necessary
- Provide support and conflict mediation.

OTHER ACTIVITIES:

- Provide feedback about housing accessibility and seniors’ housing issues to other organizations as an advisory committee member.

ADMINISTRATION:

- Maintain up to date case notes, progress reports and client files with relevant and up to date documentation.
- Maintain appropriate statistics in client data base.
- Provide information for funder reports as necessary.
- Maintain client files.
- Provide letters for clients.

ORGANIZATIONAL RESPONSIBILITIES:

- Work collaboratively with other Dixon Hall staff and departments and volunteers.
- Operate in an effective, efficient and client focused manner to achieve the best outcomes for clients.
- Actively participate in department and agency meetings and events.
- Work under the guidance of the agency and program policies and procedures, including the Mission, Vision and Values of Dixon Hall and the agency’s strategic direction.
- Meet regularly with manager to discuss individual, team and the overall effectiveness of shelter client services.
- Undertake other tasks and responsibilities as assigned within the purview of the job description.
- Other duties as required.

**Qualifications**
- BSW or MSW or post-secondary degree in a related field.
- At least 2-3 years of experience supporting seniors/individuals who are homeless, at risk of becoming homeless or who are under housed.
- Familiarity with issues concerning seniors and adults with disabilities including those with mental health, disabilities and addictions.
- Proven ability in case management and supportive counseling.
- Effective verbal and written communications skills.
- Demonstrated knowledge of community agencies and their relevant programs and services.
- Self directed and independent but also proven experience in working effectively as a team member in collaboration with other staff members and community professionals.
- Understanding and knowledge of the housing and health sector and services
- Ability to exercise good judgment, flexibility, creativity, and sensitivity to changing situations and needs.
- Must be qualified to work in Canada legally for the employer.
- Able to provide a Vulnerable Sector Screening check with a ‘Clear’ or satisfactory result that meets the operational requirements of the Agency.
- Must have up-to-date vaccinations against COVID-19 with a vaccine that has been approved for use in Canada.
