The Client Intervention Worker will be required to work Monday to Friday from 9:00 A.M. to 5:00 P.M.

**Position Summary**

Reporting to the Manager, Shelter and Respite Services, the Client Intervention Worker will support the Homelessness and Addictions Recovery Treatment Hub (HART Hub), in collaboration with Fred Victor and the Downtown East Toronto Ontario Health Team (DET OHT). HART Hubs are a three-year demonstration project funded by the province with a target client population of those with complex service needs (i.e., those experiencing homelessness, substance use, mental health, social service support needs and unemployment). The Client Intervention Worker will provide comprehensive support to individuals through case management, referrals to primary healthcare, and other services.

**Duties and Responsibilities**

Case Management
- Assist clients to develop an individualized plan of action with short
- and long-term goals and identify strategies to achieve and maintain housing stability.
- Providing support, assessment, and counseling to individuals struggling with substance use or other addictions.
- Conducting thorough assessments via phone and in person to determine the nature and severity of an individual’s addiction, including identifying any co-occurring mental health conditions
- Providing individual sessions over the phone and in person, utilizing evidence-based approaches like motivational interviewing and cognitive behavioural therapy.
- Connecting clients with appropriate resources, such as detox facilities, residential treatment centers, or community support groups, and managing their cases throughout the treatment process.
- Providing information about addiction, its effects, and available treatment options as well as offering guidance on relapse prevention
- Build partnerships and collaborate with other harm reduction programs in the designated geographic boundaries of Dixon Hall programs.
- Responding to urgent situations, providing immediate support and guidance to individuals experiencing crises related to their addiction.
- Assess and assist clients to manage episodic and stable chronic health conditions associated with persistent drug and alcohol use.
- Participate in case conferences with tenants as needed.
- Escort and accompany clients to hospitals, doctors, court, etc.
- Provide educational support to clients regarding emerging and persistent challenges related to addiction.
- Assist clients to navigate the mental health and addictions services to support their success in maintaining their housing.
- Inform clients of their rights and responsibilities.

Housing Access
- Assess and identify all potential residents’ housing needs and appropriateness.
- Assist with access to financial supports to facilitate housing access
- Arrange steps for clients to move into housing (i.e., income verification, contact with property management, financial arrangement regarding payment of rent, etc.)
- Other duties as assigned.

Administrative
- Maintain appropriate information for statistics, reporting and files
- Maintain up to date case notes, progress reports and client files.
- Attend department team and agency staff meetings

**Qualifications**
- Relevant post-secondary degree or diploma in social services, or equivalent work experience in the field Harm Reduction/Mental Health Support (required).
- Minimum of two (2) years related experience in crisis support, substance use issues, mental health and harm reduction and crisis intervention and counseling skills.
- This role requires strong communication and interpersonal skills, as well as a thorough understanding of addiction treatment principles and practices.
- Thorough understanding of the Stages of Change Principles and the ability to identify each stage and support with appropriate interventions.
- Demonstrable ability in case management with supportive counseling; strong assessment and clinical skills.
- Strong written and verbal English language communication skills; fluency in additional languages will be considered an asset.
- Knowledge of the Mental Health Act, DSM IV, and psychiatric medications.
- Knowledge of best practice guidelines for disorders assessment and treatment.
- Knowledge of community agencies and their relevant programs and services.
- Self-directed and independent; ability to work effectively as a member of an inter-disciplinary team and in collaboration with other community professionals.
- Ability to exercise good judgment, flexibility, creativity, and sensitivity to changing client situations and needs.
- Experience with case management software an asset.
- Emergency First Aid and CPR certification an asset.
- Crisis Prevention and Intervention training/certification an asset.
