London Cares programming continues to grow and expand to meet the needs of the community we serve. We are seeking Housing Stability Workers to join our team in a full-time capacity working 36 hours per week, shifts will be approximately 12 hours in length and will include weekdays, weekends and statutory holidays. The current open line is for a day position.

RESPONSIBILITIES

In this role you will be a member of the Supportive Housing Team reporting to the Supportive Housing Coordinator, or any other person that the Employer may assign from time to time and carry out such duties as assigned to you by the Employer.

Duties include, but are not limited to:

- Establish and maintain relationships with participants who have experienced homelessness through the delivery of in-home intensive, ongoing and collaborative case management services.
- Provide intensive case management support to a consistent caseload where you will be the primary housing stability worker.
- Working in a team environment, collaborate with other Housing Stability Workers to provide additional case management support when needed using a strengths-based approach, support individuals and families to achieve stability in their home and community utilizing case management tools such as crisis plans, honest monthly budgets and guest management plans.
- Help them to establish personal goals and support them to achieve those outcomes through regular communication, organizing meaningful daily activities, connecting them with other services (healthcare, addiction, trauma, employment, social, etc.), and creating proactive crisis support plans.
- Foster a sense of community and belonging in a residential setting.
- Assist with various tasks associated with transitions of residency such as orientation of new tenants.
- Listen and respond to residents’ concerns, questions, and requests.
- Support with crisis management, de-escalation, and conflict resolution in a team approach.
- Teach and model life skills such as neighbor relations, cooking, cleaning, personal hygiene.
- Respond to overdoses and monitor individuals who may be under the influence of various substances.
- Perform other related activities, as necessary or as assigned.

**Qualifications**:

- Post-Secondary education in the field of Social Work, Human Services, Addictions, or Mental Health and/or equivalent experiences.
- 2+ years of experience working with individuals experiencing homelessness and vulnerable populations.
- Complex case management experience.
- Clear vulnerable police record check.
- Valid Standard First Aid/CPR is a requirement of this position.
- A valid ‘G’ driver’s license.
- Training in non-violent crisis intervention and ASSIST is considered an asset.

Skills and Abilities
- Comfortable with supporting participants in a residential living environment
- Advanced skills in motivational interviewing and facilitating SMART goals.
- Experience working with multi-disciplinary teams such as health care, housing,
- developmental and other community service agencies.
- Ability to cope with and embrace change, ambiguity, and uncertainty.
- Willingness to uphold a low-barrier approach and understanding of harm reduction and housing first principles.
- Strong advocacy and in-the-moment conflict resolution skills.
- Knowledge of community resources in the City of London and system navigation will be an asset to this role.
