Client Care Manager (6 month Contract)

I Have A Chance Support Services Ltd. strives to support and encourage people on a route to independence. Our enthusiastic and creative team is looking for a CLIENT CARE MANAGER on Contract with possibility of extension someone who is passionate about helping others.

Role Summary:

- Develop individualized program plans for clients with complex needs. This includes individuals with disabilities, addictions and behavioral needs.
- Ensure annual Person Centered Planning Meetings and monthly Person Centered Planning Reviews occur and are documented.
- Ensure consistent interpretation and implementation of Planned Procedures.
- Support and develop programming to help individuals in care focus on health and healthy living, including: medication compliance and daily living skills
- Maintain client program files and communicate with stakeholders to ensure that all client forms are signed and current.
- Review required documentation to ensure completion and accuracy regularly. Create and maintain contractual reporting
- Practice, promote, and ensure employees are aware of expectations and responsibilities of their positions through communication, coaching and debriefing as needed.
- Oversee Support Home schedules to maintain adequate staffing levels
- Participate in a rotating after hours on-call schedule as required.

**Qualifications**:

- A Post-Secondary Degree in a related field of study
- Clean Criminal Record Check
- 2+ years of relevant experience
- Strong knowledge of person-centered and group program planning as well as best practices for residential support work
- Demonstrated ability to work effectively in a collaborative, team-oriented work environment
- Ability to navigate MS Office (including Excel, Word, Outlook and PowerPoint)
- Well-developed written and verbal communication skills
- Demonstrated time management and able to meet tight deadlines
- Work well under pressure and an ever changing environment
- Strong communication skills both written and verbal

**Salary**: $52,000.00-$60,000.00 per year
